The well is equipped with 6.625 inch casing open ended with no possible way to prevent the possible escape of gas, brine, or oil. The well was venting gas during inspection and could be smelled on the road. No changes to the well have been made since the inspection by ODNR Inspector Lynn Parkins on 9/01/15. Pictures were taken to assist inspection.
0276537254 2009-05-20 Production Wells Violations Noted My inspection this day found well still idle. The former operator, Ben Smeltzer, had committed to getting this well in production back in 2004. All equipment has been removed from this well and the casing left open. It is laeking a small amount of natural gas. This well must be produced or plugged and proper ID signs posted. Well owner must submit annual production. Failure to comply may result in bond forfeiture. 1/20/12 - The operator said he plans to produce this well and has made an attempt to get the collar back on the well.
0735767604 2015-03-23 Production Wells No Violations Upon inspection, it was observed that this well is idle and incapable of production. The well consists of an open-ended 6.625 casing sticking out of the ground. The well is making a slight amount of gas. I spoke with Mr. Ben Smeltzer about this well and he claims to own it. I told Mr. Smeltzer that he needed to put the well on his bond and he agreed. He also stated that he has a well head for the well at his shop and he plans on having James Williams of Gas Enterprise help him weld it onto the casing so he can start producing the well. Mr. Smeltzer stated that he would have the well in compliance within 60 days of today.

1087406395 2004-06-16 Production Wells No Violations Talked to Ben Smeltzer - he owned this well originally, said it was drilled in 1930s - he sold it to Richard Antill, who sold it to Robert Armstong - he owns it now and is called Heft #3 Old Well -- will be producing soon.
1583781468 2015-09-01 Production Wells No Violations Upon inspection, it was observed that no change has been made to this well since the last inspection on 3/25/2015. The 6.625 casing is still open to the atmosphere with no production equipment at the surface. There is gas escaping to the atmosphere. I called Ben Smeltzer and left him a voicemail stating that, at a minimum, a swedge with a valve and bull plug must be installed on the well as soon as possible.
1728929012 2004-06-10 Orphan No Violations Inspection this day found very old well - casing sticking out of ground - no cap, no swedge - I dont believe this is 8250 but if you could find out more info on 8250, maybe I would agree. Was 8250 ever drilled? This hole making some gas, G. Depuy would like to adopt if available. Needs plugged or capped - odor when drive down the road.
