0694288205 2017-11-28 Production Wells No Violations I conducted a follow up inspection regarding the removal of contaminated material from a tank pad and installation of a replacement tank as detailed in Compliance Notice # 2070607660. My inspection revealed that the spoils pile was still present and secured under a pit liner. A replacement tank was not on location. I spoke with Jeff Moore ( Moore Well Services ) to inform him that the compliance date was 40 days overdue. Mr. Moore said, that He has been derelict in pursuing the contractor but will get it done asap. The Compliance Notice # 2070607660 remains in effect.

1557485013 2019-03-28 Production Wells No Violations At the time of my inspection the well is equipped with an electric pump jack unite. The production valves were open at well. The tank battery consisted of one 100-barrel production tank, a mechanical separator and one meter run with a current chart. The load line to the tanks production valve was secured with a plug. An earthen dike surrounded tank battery. Proper identification was posted at the tank battery. The lease road was in stable condition. Production records are current. Compliance issues have been taken care of noted in pervious inspector report.
1574751936 2019-01-08 Production Wells No Violations A follow inspection was conducted to assess the level of compliance with Compliance Notice # 2070607660 regarding a leaking tank. I found that the contaminated soil was removed from location and a 100 barrel stock tank had been installed. The I. D. signage was in place and the earthen dike was re-established. All compliance issues had been corrected.
1893983405 2017-10-23 Production Wells No Violations I spoke with Jeff Moore ( Moore Well Services ) about the re-construction of the tank battery after removal of the leaking tanks. Mr. Moore said, that the contaminated soil and gravel will be hauled out the week of 10/30/7 and a replacement tank installed at the same time. The contaminated material was secured and stabilized under a pit liner with fresh dirt on top. It rained during my inspection and I did not see any remnants of an oil sheen in the ponding waters. Compliance Notice # 2070607660 remains in effect.
2070607660 2017-10-04 Production Wells Violations Noted The tank battery was accessed on a graveled road. The wellhead did not have a road and was located in a pasture. The well was operated as an electric pumping well. The well was operating during my inspection. The well was surrounded with a woven wire fence. I did not identify any leaks or contamination at the wellhead. The tank battery was comprised of one (1) - 210 barrel stock tank, one (1) - 100 barrel brine tank and one (1) - 2408 separator. The posted I D signage was not legible and a sign reflecting the previous Operator was still on the 100 barrel tank. The load lines were plugged. I found a large L shaped brine scald area approximately 35 X 20 on the north and west side of the 210 barrel and 100 barrel tanks. The scalded area migrated into the adjoining pasture field. Further inspection revealed that the 100 barrel and 210 barrel tank bottoms had lost integrity and were presently leaking brine onto the gravel pad. The 210 barrel tank had previously been patched with a fiberglass material. There was a pile of wood chips adjoining the scalded area that appeared to be used in the clean up. I spoke with Jeff Moore ( Moore Well Services ) and requested that both tanks be immediately drained down the two (2) inch outlet and to shut in the well until compliance has been achieved. The Owner / Operator was in violation OAC 1501:9-9-05-
2100081459 2017-10-05 Production Wells No Violations Moore Well Services (MWS) shut in the well and disconnected the 210 barrel stock tank and the 100 barrel brine from the two (2) inch piping, opened the inspection hatch and drained the fluids from the tanks. MWS placed the tanks on pit liner material for containment while steaming out the tanks prior to removing for disposal. The contaminated soil and gravel was excavated down approximately 18 to 24 to an impermeable yellow clay horizon of the Hiram Till Series. The contaminated material will be stabilized and covered until transported for disposal. The Compliance Notice was not satisfied at the time of my inspection.
