0463593039 2015-12-11 UrbanProduction Wells No Violations I inspected this producing well as part of routine inspection activity. The well and associated tank battery are located in a maintenance area at the country club facility. At the time of this inspection, the enclosure gate at the wellhead is unsecure. The lock hasp is missing. The battery gate is locked and secure. Load lines are locked. Fittings and connections at the wellhead are secure and free from signs of leakage or problems. Areas around the wellhead and battery are comprised of grassy vegetation and mulch piles. I observed the following maintenance deficiency: Ive notified Mark Smith with PEP Drilling Co. about the lock deficiency.

1706545905 2014-10-15 UrbanProduction Wells No Violations I inspected this producing unit as part of routine inspection activity. This well was completed on 03/04/2007. The wellhead and battery are located within the Beachmont Country Club facility on Chagrin Blvd. Access to the wellhead and battery are from the multi-use driveway shared with the facility. There is no dedicated access road. The wellhead is fitted with a mechanical plunger lift system and is located in a locked and gated chain-link enclosure. The fittings and connections are intact and appear functioning normally. The tank battery is comprised of 2 100-barrel low-boy production tanks, 1 100-barrel brine tank, and separator unit in a chain-link enclosure. The interior areas are in good condition and free from signs of leakage or problems. Spacing of elements meets requirements. Process safety gear (spark arrestor, low-pressure relief valve, carbon filter) are present. The secondary containment is intact and free from fluids. Enclosure gates are locked. I did not observe any problems at the time of this inspection.

2022515111 2017-05-30 UrbanProduction Wells No Violations I inspected this well location as part of routine inspection activity. At the time of this inspection, I observed the following: The wellhead and tank battery enclosures were locked and secure. Tank lids were closed and sealed. Load lines were closed, plugged, and secured with locks. The secondary containment was intact and dry with no signs of leaks or spills. Fittings and connections at the wellhead and battery were secure and free from leakage or problems. I walked the areas surrounding the wellhead and battery which are comprised of meadow grasses and lightly wooded areas, and found no evidence of problems or impact to the surrounding greenscape attributable to the on-going production activity. I noted that the wellhead enclosure was lacking ID signage and I notified Mark Smith with PEP Drilling.
