At the time of this inspection, the well was not in production and the valves on the flow line was closed. No updated production records have been filed for this well and the last reported production was in 2011. There was an audible leak around the stuffing box. The stuffing box was covered in oil and old oil staining around the well casing was observed. The tank battery still had a tank valve that was dripping brine and there was water surrounding the 210 BBL production tank. I conducted a high range Quantab test of the water and it maxed out the Quantab. The tank battery was diked and the water was contained within the diked tank battery. No signage has been added to the well or tank battery locations. I contacted Charles Moseo about the leaking tank valve and to repair the leak at the stuffing box on the pumpjack. I also requested that Mr. Moseo remove the water from the tank battery location. |

1931418690 | 2015-04-22 | Production Wells | Violations Noted | Complaint# 5010, Jake Ward. ODNR Division of Oil & Gas records show this well has not been drilled. My inspection found the well is equipped with a pumping unit with all the equipment in tact to produce the well. There is a 1100 gallon plastic tank five feet north of pumping unit which was one quarter full of fluid, also there was another 1100 gallon plastic tank located ten feet east of pumping unit which was one half full of fluid. There was oil stained vegetation east of pumping unit in a 30X10 area. The oil has not flowed from area. There is no identification posted at the well location. This well shares a common tank battery with permit number 1926. The tank battery is equipped with one 210 bbl, one 110 bbl steel tanks and one separator. The 100 bbl tank is dripping fluid from the unloading valve at the base of the tank on the east side. I tested the fluid that had pooled under the valve with a 1176 quantab. The result of the test showed the chloride level was over the maximum limits of the quantab [6400 ppm ]. There is no identification posted at the tank battery area. The well is not in production at this time. The last reported production for this well was 54 bbls of oil, 865 mcf of gas and 141 bbls of brine in 2011. There is a unresolved Chiefs Order for this well for failure to maintain liability insurance issued on 7-14-2014. REMEDIAL WORK REQUIRED TO RESOLVE THE VIOLATIONS: REPAIR LEAKING TANK IMMEDIATELY, OBTAIN LIABILITY INSURANCE, UPON OBTAINING LIABILITY INSURANCE PUT WELL BACK INTO PRODUCTION, REMOVE OIL STAINED VEGETATION AND SOIL FROM WELL LOCATION, ESTABLISH IDENTIFICATION AT THE WELL AND TANK BATTERY LOCATIONS, SUBMIT PRODUCTION REPORTS FROM 2012 TO PRESENT, SUBMIT WELL COMPLETION REPORT FOR THE WELL. NOTE: I found no oil on or near the pond on the property. I spoke with Katie Brainard [ landowner ], she told me there was no problems with her water well.
