0039714742 | 2017-10-30 | UrbanProduction Wells | No Violations | I conducted a reinspection of this well site as a follow-up from the last inspection on 9/15/2017, and involving complaint #5701. The dike has been reworked and increased in capacity. The large stones were removed from the dike and rebuilt with soil. The property adjacent to the dike, was observed to not have been disturbed or impacted. The 100 BBL production tank had been repainted and new signage was installed at the tank battery location. No violations observed. |
0544851245 | 2013-10-28 | UrbanProduction Wells | No Violations | My inspection found the well to be equipped with a plunger lift unit. The tank battery consists of a 100 BBL production tank, a 25 BBL poly brine tank, and a mechanical separator. There is no identification on the location. The dike around the tank battery needs to be rebuilt. The lease road is in stable condition. I will contact the company about the issues. |
1510054339 | 2017-03-20 | UrbanProduction Wells | No Violations | During this inspection, the well was observed to be equipped with a plunger lift assembly and the valves on the wellhead were open. Production records for this well were up to date. The tank battery consisted of one 100 BBL production tank, one 25 BBL steel brine tank and a mechanical separator. Valves on the separator were open and the chart showed production. Tank valves were equipped with flat plugs. The dike around the tank battery needed to be rebuilt due to weathering. Signage at the tank battery was not legible and needs to be reinstalled. The lease access was stable. I contacted Richard Campola of Select Energy Investments (440) 759-3506 regarding the need for rebuilding the dike and reinstalling new signage at the well site. |
1614216248 | 2017-09-15 | UrbanProduction Wells | No Violations | I conducted this inspection regarding complaint 5701 from landowner Dean Eppley. Mr. Eppley stated that the dike had been reworked but was not large enough for containment and the work that was conducted had negatively impacted his property. At the time of this inspection, the tank battery dike had been reworked but was full of medium to large sized rocks. The area of land surrounding the dike, was observed to not have been disturbed or impacted. Signage was still needing to be replaced at the tank battery location. I contacted Richard Campola, owner of Select Energy Investments about reworking the dike to remove the stones to make the dike impermeable. I also discussed the dike size and that it would need to be larger in order to meet requirements for containment, and install new signage at the location. Mr. Campola indicated that he would be back out on site within the next two weeks to address the above issues. |
