Inspection Data

Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
1342396646 2012-07-15 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 9 5/8 intermediate casing. Nomac Rig #41 reached a total depth of 2012 and ran 47 joints of 40#, 9 5/8 casing to a depth of 2012. They installed 17 centralizers on this casing string. Halliburton cemented the 9 5/8 casing with 750 sacks of Class A cement with 1/4 #/sack Floseal and 12% salt. Good circulation throughout the job and 22 barrels of cement circulated to surface.
1343056281 2012-07-23 Drill / Deepen / Reopen No Violations I was on site to check the status of Nomac Rig # 41 and check the status of Express Energy Services conductor rig drilling the other Elwood wells conductor holes. My inspection revealed that Nomac #41 is drilling the curve at 8203. Express Energy Services is drilling the conductor holes and mouse holes for the # 8 and # 10 wells. Shawn with Express Energy Services advised that it would be later today or this evening before they were ready to cement the conductor on these two wells.
1690206565 2012-07-13 Drill / Deepen / Reopen No Violations I was on site to witness the cementing of the 13 3/8 casing. My inspection revealed that Nomac Rig #41 has drilled a 17.5 hole to a total depth of 573 and ran 573 of 13 3/8 casing with 6 centralizers installed on the casing string. Halliburton cemented the 13 3/8 casing with 540 sacks of Class A cement (1% CaCl and .25 #/sack Floseal). There was good circulation throughout the entire job and there was 24 barrels of cement circulated to surface.
1879312106 2017-05-15 Preliminary Restoration No Violations

I was on location to conduct a preliminary restoration inspection. The #3H well is in production and consists of a plunger lift unit. Jersey barriers were installed around the wellhead. The well is identified with a legibly-marked well identification sign. Identification was posted at the tank battery which consisted of eight 400 bbl production tanks, three labeled as containing produced water, and four labelled as containing condensate, and one low-pressure vertical separator. The tank battery is surrounded by a dike. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Production equipment on the well pad consisted of: 6-producing wells 6- 400 bbl production tanks, three labelled as containing produced water, and three labelled as containing condensate 6 sand separators 6 gas processing units 1 low pressure vertical separator 1 vapor recovery unit 1 compressor station 1 line heater 2 bullet tanks Jersey barriers are installed around the production equipment. The access road and man doors were locked. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road was stable in in good repair.
