2847892 Primary Facility 2019-03-04 Routine/Complete Inspection No Violations Noted The main access road entrance for this well location is off of Hull Hill Road, in Brokenstraw Township, Warren County, PA.

A representative for the operator is currently on site. Permits for this well, along with several other wells in the area were recently transferred to the current operator. This is a combination oil/gas producing well, fitted with an electrically driven pump-jack. The well is currently being treated with nitrogen. There is no evidence of any major spills or releases around the well head. The API ID is clearly posted, along with the previous operators contact information, inside the electrical service box of the pump-jack. The location and accessing road are currently frozen and snow covered. There are no evident E & S or stabilization related issues.
