Inspection Data

Inspections Performed

Inspection ID Inspection Category Inspection Date Inspection Type Inspection Result Inspection Comments
1822400 Primary Facility 2009-08-20 Routine/Complete Inspection No Violations Noted The silt fence located beside the access road has been overtopped with stone. The fence should be addressed to reduce the risk of off-site transportation of sediment. Well is in pre-drill phase. The operator should also place some silt fence around the top soil stockpile or temporarily seed and mulch it. The operator should continue to monito the access road and well site for erosion and sedimentation until final restoration activities are completed.
1824637 Primary Facility 2009-08-27 Drilling/Alteration No Violations Noted At the time of inspection, no E&S or IW issues at the site. Pit aligned with 20 mil plastic and no freeboard issues. Falcon drilling currently at 2850¿. Falcon set 13¿ conductor pipe at 43¿ and 7¿ at 1231¿and used 160 sks class A cement. Water noted at 395¿. TD expected at 3675¿. Well sign and E&S plan located at the entrance way of site.
1824652 Primary Facility 2009-08-19 Routine/Complete Inspection No Violations Noted Well pad was currently being constructed. Well site marked with a stake. E&S issues constructed. Pit aligned with 20 mil plastic. Called the DEP WQ personnel for assistances at the site. Water not draining from the access road. No drainage area along the road and entrance way had not collection are or drainage area. Water lying in front of access road and mud on the road coming from this well site.
1833239 Primary Facility 2009-09-30 Routine/Complete Inspection No Violations Noted At the time of inspection, the well was drilled. The brine tank was in sound condition, surrounded by a dike. Pits were in good condition with fluids removed; plans are to solidify pit upon approval. Access road was in stable condition with functioning BMP¿s (ie. filter fence). No accelerated erosion or sedimentation was observed in relation to the site or access road. Noted a 6 inch field ditch with pipe was being installed to the left hand side of the access road.
1925531 Primary Facility 2010-11-03 Site Restoration No Violations Noted At the time of inspection, well drilled and in production. Site adequately and uniformly restored with >70% vegetative growth, site had been regraded to preexisting contours and land uses. No accelerated erosion or sedimentations issues were noted. Acces road stable. Brine tank sound surrounded by dike. Observed a large pile of rock onsite in front of well, appears operator may use in roadway ditches.
