0113901535 2018-11-27 Production Wells No Violations On this day, I am accompanied by ODNR Southwest Supervisor Eric Conrad. Utica Resources Operator, Jimmy Reese, was on location. The site consists of (2) producing wells and (2) gas processing units on a gated pad within an earthen dike. The production facility is off the pad, located near the gate, and consists of (4) 400 barrel steel tanks and (2) 500 barrel steel tanks all within steel containment with a liner. Annular pressure monitoring devices are equipped and labeled on each wellhead. The 13.375 inch diameter casing gauge has a pressure of 410 pounds per square inch, and the 9.625 inch diameter casing gauge has a pressure of 410 pounds per square inch. Fencing encompasses the wellheads, and proper identification was requested to be placed on the individual wells.
0433211362 2019-05-14 Preliminary Restoration No Violations On this day, I am conducting a preliminary restoration inspection. Site meets ORC 1509.072(A). The pad is gated with signage. There are (2) producing wells, (2) gas processing units, (2) 500 barrel steel tanks, and (4) 400 barrel steel tanks in containment. The wells are surrounded by livestock fencing and have annular pressure monitoring devices equipped. The 9.625 inch diameter intermediate casing has a pressure of 440 pounds per square inch (psi), and the 13.375 inch diameter surface casing has a pressure of 440 psi. The production records are current. It will be noted that I spoke with Utica Resources Representative Ryan Weisenbarger onsite.
0555239186 2016-01-21 Administrative Inspection No Violations Received call at 9.59 am from PDC Company Man Dave Gray stating they pressure tested the 9.625 casing after remedial cementing. Mr. Gray stated they pressured to 792 psi and after 30 minutes of hold time the pressure leaked to 741 psi, indicating a successful pressure test. Mr. Gray also stated a cement bond log was ran and cement top is now at approximately 350 from surface.
0985240203 2019-05-06 Production Wells No Violations I was on location today to do a field inspection on this well. At this time the well does appear to be producing out of the tubing wellhead and the casing wellhead. There are no violations at this time.
1065282188 2016-05-11 Drill / Deepen / Reopen No Violations On location to witness hydraulic fracturing operations for this well. Halliburton is the completion contractor. Witnessed the pumping of the toe stage, which was pumped through 3 toe sleeves with large diameter holes. The formation broke down at 8753 psi. Treatment pressures ranged from 5900 to 8990 psi with an average rate of 80.6 bbl/min. 49806 lbs of 100 mesh sand and 351664 lbs of 30/50 mesh sand was pumped into the formation. A gel flush was run before shutting the well in. The initial shut-in pressure was 3463 psi and the 5 minute shut-in pressure was 3061 psi.
1085325612 2016-05-17 Drill / Deepen / Reopen No Violations On location to witness stimulation operations for this well. Upon arrival, Halliburton was pumping stage #22 of the frac job. Formation break pressure was 7464 psi and treatment pressures were staying steady near 7360 psi with a rate of 80.0 bbl/min. The initial SIP was 4598 psi and the 5-minute SIP was 4055 psi. A total volume of 5605 bbls of freshwater and 403122 lbs of sand were used during this stage.
1118444746 2016-05-26 Drill / Deepen / Reopen No Violations Stimulation operations for this well are now complete. Halliburton has moved completions equipment off the location. Construction of the tank battery and pipeline tie-in have begun on the location.
1186809177 2015-11-23 Drill / Deepen / Reopen No Violations On location to pick up cement tickets and check status of rig-down. Mr. Dave Gray of PDC Energy stated that the 12.25 hole section was TDd at a depth of 5588, the hole was loaded with saltwater, and the well was temporarily capped. A larger rig will be brought in this winter to finish drilling the deep intermediate section, run/cement 9.625 casing, and drill the lateral section. Solidification of cuttings is taking place on the pad today.
1289111382 2016-05-10 Administrative Inspection No Violations Matt Wolf of PDC Energy called at 11:07 am to notify that Halliburton is moving onto the Mason Pad to begin stimulation operations. They will start with the 2-H and will perform a 27 stage frac before moving to the 4-H. Operations are scheduled to begin at 9:00 am on 5/11/16.
1377630604 2016-11-15 Preliminary Restoration No Violations The purpose of my inspection today is a status check on a preliminary restoration. During my inspection the well was producing and is equipped with a production tree wellhead. The production equipment consists of six 500 barrel production tanks, two glycol dehydration units, and a flare stack. Production equipment is shared with 4H 3416729759. The pipeline tie in located by the lease road has been completed and the location has been resorted. The well pad has a contamination free, gravel surface, with no erosion issues. All drilling equipment has been removed from location. Location has met 1509.072 A requirements.
1447680697 2015-11-14 Casing String Witnessed No Violations I was on location to witness the surface pipe being cemented back to surface. We had 35bbls of cement returns back to the surface. The plug was bumped to 800lbs and held with no leaks. Inspector LJ Parkins will be doing the paperwork on this well.
1453475008 2016-01-19 Administrative Inspection No Violations Received a call at 10:48 pm from PDC Drilling Engineer Blake Roush stating a cement bond log of the 2nd stage for the 9.625 intermediate casing showed a cement top of 2158, which is well below the casing seat of the 13.375 intermediate casing (1688). Mr. Roush stated that PDC plans to perforate the 9.625 casing at 2050-2054 and perform a remedial hesitation squeeze cement job. Mr. Roush stated the cement will be spotted through 4.5 drill pipe with a packoff installed to create pressure. The DV tool is at 3908 and has been pressure tested successfully.

1533781838 2016-01-19 Drill / Deepen / Reopen No Violations On location to check status of drilling operations and receive cement tickets for the deep intermediate string. PDC Company Man Mr. Larry Howell stated that circulation was lost toward the end of freshwater displacement for the 2nd stage of the cement job. No cement returns were seen at the surface. Mr. Howell stated that PDCs first course of action will be to attempt tagging the cement top in the 9.625-13.375 annulus from above with a string of .75 tubing. If cement is tagged, it will be topped off from the surface. If cement is not tagged, a bond log will be run to determine if cement reached the inside of the 13.375 intermediate casing. Any further course of action will be determined after cement top is found. I was accompanied on this inspection by DOGRM Geologist Jake Glascock.
1637267016 2015-11-16 Casing String Witnessed No Violations On location to witness cementing of 13.375 Berea intermediate string. Universal Well Services was pressure testing their lines upon my arrival. 13.375 casing depth is 1688 and the 17.5 hole depth is 1770. There is 1 cement basket at 1057, above the Big Injun. Universal established circulation 150 bbls into pumping, conditioned the hole with gel/LCM, and pumped 738 sks Gas Block 14.5 lb/gal lead cement with 2% CaCl2, 0.15% CR-3 and .25 lb/sk celloflake. Universal then pumped 378 sks Gas Block 15.7 lb/gal tail cement with the same additives. Cement was followed by 259 bbls of freshwater displacement. The wiper plug was landed in the float collar and pressure was increased by 500 psi and held for 5 minutes. The pressure test passed successfully with minimal pressure loss. The float collar held after releasing pump pressure.
1951045057 2016-01-20 Drill / Deepen / Reopen No Violations On location to witness remedial hesitation squeeze cement job on 9.625 interemediate casing. This objective of this job is to extend adequate cement bond at least 200 into the 13.375 intermediate casing, however enough cement will be spotted to reach surface with 30% excess. Mr. Blake Roush stated that the method of spotting cement has changed to a bullheading technique through a 9.625-2 swedge with a valve installed to lock in pressure. I witnessed Universal Well Services establish good circulation through the perforations (4 holes, 0.5 diameter, 2050-2054) and spot cement downhole at 1850. PDC will allow cement to thicken for 1.5 hrs before attempting to pump cement closer to the perforations and squeeze it into the annulus. I had to leave location for another job at 11/30 am.
