-55, 94 lbs/foot. Casing was centralizer with 2 centralizers. Shoe track is 43 feet long. Previous casing set was a 30 inch diameter conductor to 100 feet. Cement Baskets were set at 510 feet. O-tex is onsite with 860 sacks of Class H cement. 50% excess ran in open hole. Bulk cement was mixed and weighed on mud scale but when cement was at 15.6 ppg it was too thick to pump. Another bulk truck was ordered and job was completed later in the day. Cement to surface was not witnessed. Mix water has pH of 9, 132 ppm chlorides and 68 degrees Fahrenheit.

0676202787 2019-03-28 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well site meets ORC 1509.72 with the area affected by the well site construction restored, vegetation is established and erosion control is in place. The well pad surface and approach is stone construction and in good repair. 5 wells are present on the pad and in production, all wells are selling off the 5.5 production casing through (2) primary master valves with emergency shut downs in the production outlet flow line. The wells are selling through sand trap separators with a temporary choke system attached to the sand trap separator. The sand trap separators are being evacuated into a single 500 barrel steel horizontal tank set in polymer containment. Permanent production equipment includes (5) gas processing units along with a production facility consisting of (6) steel 400 barrel tanks set inside a polymer lined steel containment dike area. Also present is a 3rd party mid stream facility.
