2585706 Primary Facility 2017-04-20 Incident- Response to Accident or Event Violation(s) Noted On 4/20/2017@9:45 Operator discovered a 6bbl oily brine production fluid spill. Operator reported this to the Department on 4/20/2017@11:25 (CTS ID 326356).

On 4/20/2017@12:10 Department contacted Operator (Spencer Johnson) who stated that spill was caused by the cattle breaking through the fenced well site and compromising the line and valve from production equipment to brine tank. Spill contained to earthen dike and clean-up was being mobilized.

On 4/20/2017@12:30 Department performed inspection. During the inspection, no active leaking was observed, Department able to visually confirm the reported oily brine spill. The spill appears to be contained to within the earthen dike. At the time of inspection, clean-up work had not commenced and no containment measures surrounding the impacted area was observed. Department spoke with Operator (Spencer) to recommend Operator perform cleanup in accordance with Spill Policy and to request Operator test for all constituents of brine and petroleum hydrocarbons. 2588331 Primary Facility 2017-04-26 Follow-up Inspection Outstanding Violations - Viols Req'd Inspection performed as a follow up to inspection ID 2585706. At the time of inspection clean up appears to have commenced. Impacted area has been excavated and backfilled with gravel. No E&S issues. Department recommends Operator continue to perform routine monitoring and maintenance to ensure clean up in accordance with spill policy and forward sample test results upon receipt.

Previously issued violation remains outstanding.

2606098 Primary Facility 2017-06-20 Compliance Evaluation No Violations Noted At the time of inspection clean-up appears to have been completed. Earthen dike spill area has been backfilled with new gravel. Compromised brine tank valve has been fixed. No E&S issues.

Department awaiting final remediation report from Operator. Department will follow-up to ensure adequate restoration is maintained.

2615076 Primary Facility 2017-07-13 Routine/Complete Inspection No Violations Noted At time of inspection well appears to be in production. Pump jack located on site. Well head surrounded by security fence.

An operator reported release of production fluid occurred on 4-20-17. The NOV was closed on 6-20-17. The area of the release has been rocked. Department is awaiting final remediation report from operator. A follow up inspection will be conducted at a later date.
