2077119 2012-06-11 In Compliance with Policy 06-11-2012 John Ley, WQS SWRO was on site and completed an inspection of the facility. The material had been cleaned up and disposed of properly.

06-19-2012 In response, ESC&R included: 1) a summary of the incident in the paragraph above and a copy of their incident investigation form; 2) a copy of their bill of lading; 3) verification that all remaining vac boxes were removed from the site; and, 4) a memo from ESC&Rï¾’s EHS coordinator to their staff regarding safe guards to limit the chance of this type of incident from occurring in the future.


2100356 2012-09-18 No Violations Noted On May 3, 2012, WQS John Ley, Bill Keefer and I stopped at the Don Pletz residence to investigate a possible private water supply impact by gas drilling activities. The Pletz residence is located at 1556 Blackhawk Road in Darlington PA and the owner lived there about six years. The well is located west of the residence at N 40ï¾° 45ï¾’ 46.9ï¾”, W 80ï¾° 29ï¾’ 25.9ï¾”.This location is in the floodplain of Brush Run Creek, an ARD-impacted stream from strip mining

The water tested positive for total coliforms indicating surface water influence. There were no hits at all in the VOC sample. The inorganic results were not good. TDS, sulfates, sodium, chlorides, alkalinity and turbidity were all high. The closest well, at the time, is nearly a mile and a half away, well outside of the Presumption Area. The inorganic tests indicate the water isnï¾’t very good but without a pre-drill test it is impossible to prove any impact from the Powell unit or any other O&G wells.

2165183 2013-04-30 No Violations Noted Well was drilled. During inspection workers were installing a flow line from the brine tanks to production equipment. No erosion or sedimentation issues were noted.
2205908 2013-09-17 No Violations Noted This well was drilled and had regulatory inactive status at time of inspection. No leaks noted around brine tanks. Cellar was filled with water that did not have sheen but did appear to have substantial algae growth. Temporary stabilization appeared to be good. No erosion or sedimentation issues were noted.
