No freeboard or E&S issues at the time of inspection. Conductor pipe was installed to 22 feet (9 5/8¿ pipe) and sanded in. Surface pipe was installed to 397 feet (7¿ pipe). Surface pipe was cemented with 75 sks of Class A with 8 bbls return. Water was encountered at 458 feet (2 gpm flow). Well was drilled to TD at 1,068 feet. Production pipe was not set. Phillip Drilling was waiting on Superior¿s wire line truck to log hole. Phillip Drilling moved to Glass #6 well.

2019512 2011-11-08 No Violations Noted At the time of inspection, site Id sign,ES plan and valid permit present. ES plan has been exposed to the weather; therefore, department recommends it be replaced and also placed in a bag or other such device that will protect plan from becoming "wet."
Recommend operator add additional rock to shared access road due to muddy conditions, operator shall also continue to closely monitor and maintain in order to prevent mud from reaching township roadway. Brush truck is available to ensure township road is kept clean. 

Well drilled, not yet equipped for production. One below grade pit located onsite-lined with adequate freeboard. Bmp's installed according to plan; although noted that various "large soil clumps" observed oustide of bmp's and LOD; therefore, it is recommended that operator remove this soil and continue to closely monitor and maintain all bmp's until final permanent restoration is achieved. No accelerated erosion and sedimenation issues noted. No violations noted during this inspection.
