1184071688 2007-06-20 A follow-up inspection revealed that the well is in-compliance. C&P has been removed and the well has been placed back into production. Mrs. Gene Schmelzer was informed of the inspection results. Mrs. Schmelzer was also informed that a inspection would be conducted in 1 year to verify production. She stated that the well did not make much oil or gas and would provide production records or a trip ticket at the time of the inspection.
1325606409 2011-12-27 Well was completed and assembled. All valves were open. Electric was hooked up to the pump jack. As per the last conversation with Mark Schmelzer, the well had been fracked in October of 2011.
1584168882 2010-12-17 An inspection was requested by Central Office for the evaluation of Temporary Inactive Status. The well status remains unchanged since the last inspection. The well is idle and consists of 5.5 casing with a swage and 8 wellhead. The pumping unit along with rods and tubing have been removed, but remain at location. No observed connection into the 100 bbl tank.
-194585569 2007-04-04 A follow-up inspection revealed that violations have not been resolved. The well still appears to be idle. The electric motor is missing a belt, and electric lines are lying on the ground. Contacted Mrs. Gene Schmelzer regarding this issue and time frame needed. She was given a 1 month extension to the NOV. The complainant was contacted and informed of the inspection results.
-251812443 2010-10-28 The Division received a landower complaint alleging idle well. The landowner states no activity and requests plugging and abandonment of said well. An inspection revealed idle well. The pumping unit, rods and joints of tubing have been pulled from the well and remain at the location. A swage with valve installed with no attached flowlines from well to 100 bbl tank or house gas. Production reports indicate zero production for oil and gas since 1999. The well does not meet the requirement for commercial production under Ohio Revised Code 1509.062, which states 15 bbl crude oil, and/or 100 mcf of natural gas. History: A landowner complaint was also received in 2006. A Notice of Violation was issued for non-production. The well owner shall plug the well in accordance with ORC 1509.12; file for Temporary Inactive Status; or produce well. Failure to meet deadline will result in further enforcement action. Request well completion report submittal to the Division.

-576093456 2006-12-18 A complaint was received by the Jackson Office on 12/18/2006 alleging that a well was idle and tank battery was leaking. The complainant was contacted on 12/18/2006. An inspection was conducted on 12/18/2006. During the inspection, it was noted that the well was missing identification, old plastic gas line needs removed and a tank was leaking brine from a fitting located inside the dike. Pollution and contamination was also observed around a steel tank located near the well. Additionally, it was noted that the well appeared to be idle. Production records for this well show that the last year of production was 1998. The complainant was contacted by telephone and informed of the inspection results.

860068111 2008-03-24 An inspection revealed that this well is idle. The pump jack has been removed from the well along with the rods and 14 joints of tubing. All equipment observed on-site. Production records show that the last year of production was 1998. A Notice of Violation had been issued in 2007 for non-production. Jeanie Schmeltzer, the owner, had stated on 6/20/2007 that this well is now back into production. She was informed that this well would be revisited in 1 year to verify production. Mrs. Schmeltzer will be contacted regarding the plans for this well. An inspection will be conducted again in June 2008.

992184876 2010-11-08 Returned call from pumper, Mr. Danny Baker at 7:36 am. Mr. Baker states they may frac this well, but also wanted to know if they could place well back into prodution within 30 days. He was informed that Temporary Inactive Status might be an option because, the well does not meet commercial production and this would allow for more time to perform work. He was informed that the Division received non-production complaint 4 years ago; statements of production still show zero; and referenced Jeanie Schmelzer's previous statement about this well.
