| The producing unit consists of a wellhead with pump jack and above-ground storage tank (AST) battery. The battery is comprised of  2 140-barrel steel production tanks, 1 50-barrel brine tank and separator unit plus Dominion metering run. Both the wellhead and battery are enclosed in separate chain-link fences. The gates and load-outs are locked. ID placard and secondary containment are good. Process safety gear (spark arrestor, low-pressure relief valve, carbon filter) is present. No problems noted.

| I inspected this producing unit as part of routine inspection activity. The wellhead pump jack system enclosure is secure. Valves and connections are secure and free from signs of leakage or problems. At the time of my inspection, the area is covered with several inches of snow.  Access to the wellhead and battery are via a dedicated access road which has been plowed and is passable. Valves and connections at the battery are secure. Load-outs are bull-plugged and locked. I did not observe any problems at the time of this inspection.

| I inspected this well as part of routine inspection activity. The well is producing and production records are current. Entry gates are locked and secure, at both the wellhead and battery. Process safety equipment is present. Fittings and connections are secure and free from signs of leakage or problems. Tank hatches are closed and secure. The access road is dry, level, and well drained with no signs of erosional deficit. At the time of this inspection, I observed the following deficiencies: The electrical box is not lock protected, ID signage is lacking required information. There is no evergreen screening which is required as part of site restoration, unless waived by the landowner. Warning signage is incomplete.  I have notified Bass Energy that these deficiencies must be corrected as soon as practicable or by 06/29/2015.  Please contact Inspector Stephen Tompkins at 330-907-4647 for questions regarding this inspection.

| My visit to the well today was twofold. The city of North Royalton had requested that Bass Energy relocate the access road to this well due to construction on city property adjacent to the original access road. Bass Energy notified me that this was accomplished in late August of 2012. The access road was constructed to best management practices for site construction and meets requirements of 1509:09-9-9-05. I conducted a well inspection while on location. Well head location is clean and in good operating condition. There is 180 Pounds pressure at the well head to the feed line. The tank battery has two (2) 140 Barrel storage tanks and caps are secure and in good condition. There is one  (1) fifty barrel brine tank. There is a separator unit and a gas feed line unit. Fencing is chain link eight feet tall with three strands of barbed wire surrounding the top. Well is in compliance. No violations found.
