| Wellhead and tank battery identification signs comply with production well and tank battery requirements.  However Gallia County is considered an urban township based on 2010 US population census.  I have provided with this compliance notice the urban regulations information necessary to complete the following tasks in order to conform to the Urban population rules.  This well and its tank battery identification signs do not meet the following requirements: 1501:9-9-05E1/1509:3D.  Under urban rules signs must meet a certain standard different from normal production wells and tank batteries.  Signs must be attached to required fencing.  Fences are missing around the wellhead/ pumping unit apparatus, which consists of a pumping unit and associated two-inch production plumbing coming from the wellhead, and tank battery, which consists of two 100-barrel tanks, an oil/gas separator, and a couple of small knock out tanks.  According to 1501:9-9-05E2/1509:03D, fencing specifications must be followed surrounding the tank battery and wellheads.  Tanks must also be checked to ensure they are in compliance with 1501:9-9-05E4/1509:03D.  Locks must be place where specified according to the regulation 1501:9-9-05E3/1509:03D.  Wellhead is leaking gas at the wellhead packing between the intermediate casing and the production tubing.  The PhD6 measured 0.45 meter reading units of methane, CH4.  According to 1501:9-9-05(A)1/1509:20, owner shall repair or replace equipment to prevent the escape of oil and/or gas.  Examination of the tank battery revealed there are two breeches in the dike wall that would permit the escape of oil, brine or other wastes from the containment area in the tank battery.

1364309431 2013-03-25 Wellhead and tank battery identification signs comply with production well and tank battery requirements.
1369946885 2013-05-30 The operator has corrected the leak at the wellhead. The operator has also beefed up the dike walls around the tank battery.
1378991570 2013-04-25 Operator has reported 2012 production for this well. Violation is closed.
1446867771 2013-06-17 This well is currently not producing. According to 1501:NONE_ /1509:.062A if there is no production, the well shall be plugged or placed in temporary inactive status until production problems have been resolved. This well was also initially cited Urban Rules violations. These violations have been removed at this time
-1629540775 2004-02-13 Could not locate after lenghty thorough search. Will ask company to take me to the well.
