I gave him the regestration requirements for ownership. I explained the insurance he would have to get. After talking to his insurance agent Mr. Smith decided to not take the well and now wants the well plugged.

| I called Energy Search (EOG) Gary Smith (724) 743-2722 at 10:25am on 12-8-2008. I talked to Gary about the status of the Cennamo-Blankenship 1-10 well. This well did not transfer during the North Coast Energy buy out of Energy Search last year. This well had a current plug permit issued at the time of sale. Gary said that the well should have transferred to North Coast. I told Gary that The well needs to be capable of production or plugged. I informed Gary that the well has outstanding violations. Gary said that he will make some calls and call me back on 12-9-08. I gave him my cell number and office number.

1219398554 2007-07-10 Meeting at well site 9am with the landowner Allen Smith. Allen was thinking of buying the well from Energy Search. He would be using the gas to heat a commercial business. I gave him the regestration requirements for ownership. I explained the insurance he would have to get. After talking to his insurance agent Mr. Smith decided to not take the well and now wants the well plugged.
1228750023 2008-12-08 I called Energy Search (EOG) Gary Smith (724) 743-2722 at 10:25am on 12-8-2008. I talked to Gary about the status of the Cennamo-Blankenship 1-10 well. This well did not transfer during the North Coast Energy buy out of Energy Search last year. This well had a current plug permit issued at the time of sale. Gary said that the well should have transferred to North Coast. I told Gary that The well needs to be capable of production or plugged. I informed Gary that the well has outstanding violations. Gary said that he will make some calls and call me back on 12-9-08. I gave him my cell number and office number.

1424610964 2009-01-13 No identification on wellhead. Well is not capable of production. There are no production lines or surface equipment on location. Well consists of 8 5/8 surface casing, 4 1/2 production casing, 2 tubing and valves. All valves are shut off. Well is not open at the surface. This inspection was to verify the well status, in order to continue enforcement action to get the well in production or plugged. Pictures were taken of the wellhead. I talked to the landowner Allen Smith on the phone and he stated to me that he is not interested in owning the well at all. He stated that he wants the well plugged. I have verified with Erica Freeman (ODNR) that the Well is still under the Energy Search Inc. Bond.

Rick told me that it is on the plug list but that his boss told him to stop plugging wells and to stop spending money until the wells are sold. I told Rick that they need to write a letter if the time frame cannot be met requesting an extension. If they did not it could go to a Chiefs order.

947375549 2009-10-07 Plugging operations. 230 psi on the tubing and casing. Viking rig was flowing gas at wellhead. My gas meter signaled an dangerous level of gas near the rig. I told the rig operator to get the gas flow under control. Either by a flare located 100 feet from the well or by fluid to kill the gas. They tried using their mud pump to kill the gas flow.
