| My inspection found the well to be equipped with a pumpjack. The well is supplying domestic gas from the casing. The well and the tank battery are in good repair. The well is surrounded by a chain link fence. The tank battery consists of two 100 BBL production tanks, a 25 BBL brine tank, and two mechanical separators. The proper identification is located on the tank battery and the well. The lease road is in stable condition. The company has a compressor station next to the tank battery. The lease road and tank battery are being shared by permit number 2-0067.

| I received a call from the Middlefield township fire chief around  7:00PM, 6/1/09. The production line was leaking, getting into a ditch that was on the Swine Creek Parks ground.  I called Bob Worstall to find out a name to call.  He gave me Dallas White's number, (330-719-6359) and Oxford's superintendents name and number, Danny Glenn(330-322-0631)  I left Danny Glenn a message about the spill.  I called Dallas White.  He would go up there and see what was needed.  I inspected the site on 6/2/09.  I met with the Ohio EPA, Reginald Brown. Also there were the park service, and Danny Glenn.  The contractor Wade Pol showed up and will do the clean up work.  The fire chief could not read who to call.  I instructed Danny Glenn to rewrited the ID signs.  He did immediately.  The clean up started.  This clean up continues.
