A plastic tank is on site but not connected to well. Access to well is by a drive way and through 80 feet of flat pasture field. I did not see any evidence that any one other than the farmer has been to this location. No tire tracks or ruts were observed. No fresh marks were on any of the fittings at the wellhead to indicate that a wrench had been used recently.

| Witnessed the remainder of plugging and abandonment for Jon Scott.  The 4.5" production casing was ripped at 2768'.  Recommended to well owner/plugging company to pull casing free prior to setting the Lime plug.  Pulled 2 joints resulting in a depth of 2691'.  The plug circulated oil from the well bore.  The next plug would be heated up with CaCl2 and circulation was observed.  Also noted that oil thinned during the setting of this plug.  Circulation would again be observed on the next plug (Btm 8.625" csg).  All oil would be retrieved from the pit with liner and transferred into plastic tank.  A brush & stone plug was set at approx 100' and cement would stay at the surface.

A new gas meter has been installed. Meter has a new chart on it. Timer has fresh batteries in it. 2" valve to meter is only open 1/4". 2100 gallon tank on location. Tank is located 15' from the well. Tank has approximately 900 gallons of fluid in it. Tank is not connected to well or equipment at this time.

-749318485 2008-12-15 Identification reads Mitchell Energy. Id needs to have current owner information. Well head is shut in and disconnected from separator. Well is not capable of production. A plastic tank is on site but not connected to well. Access to well is by a drive way and through 80 feet of flat pasture field. I did not see any evidence that any one other than the farmer has been to this location. No tire tracks or ruts were observed. No fresh marks were on any of the fittings at the wellhead to indicate that a wrench had been used recently.

791564814 2009-06-23 Witnessed the remainder of plugging and abandonment for Jon Scott. The 4.5 production casing was ripped at 2768'. Recommended to well owner/plugging company to pull casing free prior to setting the Lime plug. Pulled 2 joints resulting in a depth of 2691'. The plug circulated oil from the well bore. The next plug would be heated up with CaCl2 and circulation was observed. Also noted that oil thinned during the setting of this plug. Circulation would again be observed on the next plug (Btm 8.625 csg). All oil would be retrieved from the pit with liner and transferred into plastic tank. A brush & stone plug was set at approx 100' and cement would stay at the surface.
-795930015 2009-01-14 Called Randy Moore and talked about idle wells and that they need to be either working on evaluating the wells or plugging them. Randy said that he is working on the problem. And will try to get a swab rig to evaluate the wells soon.
909289353 2009-04-27 Id is legible although it is ripped. Id is attached to old disconnected separator on site. Well is equipped with tubing and a rabbit. A new pressure regulator and drip have been installed. A new gas meter has been installed. Meter has a new chart on it. Timer has fresh batteries in it. 2 valve to meter is only open 1/4. 2100 gallon tank on location. Tank is located 15' from the well. Tank has approximately 900 gallons of fluid in it. Tank is not connected to well or equipment at this time.
-955396959 2007-07-17 Well is Idle. Well is shut in and disconnected from production lines. Has new id. Rick green told me that this well is on the plug list.
