The initial response appears to have been adequate. The spill will be monitored and the operator notified of additional MRM or OEPA requirements.

-111065960 2002-11-18 Steve Swain with Oil & Gas Solutions reported a flowline leak from the Firestone 3-B well in Bath Township to the Uniontown office. The spill volume was estimated at less than one barrel. Oil & Gas Solutions repaired the flowline and removed and stockpiled the impacted soil. The surface area impacted was approximately 30' by 10'. The site has been graded and according to Steve Swain, will be reseeded when weather permits.
1160748184 2006-09-21 This inspection was done at the request of Bath Township adminstrator Bill Snow. The valves are open and the polished rod is clean. The well appears to be in production. No detemination as to whether the well is economical to operate can nor will be made.

1415070003 2003-04-02 CASE 03-010 Steve Swain with Oil & Gas Solutions called the Uniontown office to report a stuffing box leak on the Firestone 3B well. In addition to MRM, the OEPA and Bath Fire Departments were notified. The well has an outstanding NOV for a flowline leak back in November 2002. The leak was repaired but the area needs reseeded and the stockpiled soil removed. This latest release was from the stuffing box. The impacted area is approximately 25' by 30' and extends all around the wellhead. The area is marshy with cattails, grass and standing water. The initial spill response included shutting the well in, removing and stockpiling impacted soil and vegetation, spreading peatsorb in the affected area, and laying out sorbent pads and booms. The initial response appears to have been adequate. The spill will be monitored and the operator notified of additional MRM or OEPA requirements.

18379572 2002-07-29 Status check inspection. The wellhead was in good condition. The tank battery however had at least one leaking stock tank and impacted gravel. The tank needs to be replaced or repaired and the impacted gravel and underlying soil removed and disposed.
-1956491546 2003-04-03 Follow up to check on spill remediation. The free oil appears to have either been stockpiled with the impacted soil and vegetation or stabilized with peatsorb, pads, and/or booms. I will continue to monitor the site regularly.
