| My inspection today finds Berry Well Service is attempting to retrieve 1" tubing and a packer from the well.  The tubing parted at 135' and the rig is trying to get back a hold of the tubing with a spear.  After a couple of attempt the rig crew was able to pull the tubing.

| Appalachian ran a vdl bond log.  The log showed a void from 590' up to 545'.  Appalachian perforated the 2" tubing with three shots from 590' to 580'.  The service rig ran tubing back to 1600'.  Cemenco pumped a total of 34 sks of class A cement in five 300' stages.  They then pumped on the 2" tubing and were able to push an additional 11 sks of cement.  The cement stayed at surface.  A valve was installed on the tubing to prevent the cement from moving.

I informed Mr.Dimmerling that this was actions taken due to His complaint on 04/22/13 He made to me stating the well was idle and He was going to bury it in a pond. Mr. Dimmerling demanded I stay off His property. I advised Mr.Dimmerling I had followed our standard operating procedures and policies according to the Ohio Revised Code 1509.

| Inspector Stolarik contacted me at 7:39 am and stated that he was approached by Joe Dimmerling about an idle well on his property.  I called Mr. Dimmerling at 10:43 am.  Mr. Dimmerling stated that he did not want to file a complaint at this time.  Mr. Dimmerling then inquired about how much the fine would be if he simply buried the well beneath a pond that he intends to build.  Due to that comment I decided to, along with inspector Stolarik, perform an inspection on the well.  The well is equipped with 2 tubing and has a 1 tubing string inside of it.  No sales lines are attached to the well.  There is no identification present at the well.  It appears the land owner has begun to move dirt in the area around the well.  Make well capable of commercial production within 30 days; or plugging is required within 90 days.  Failure to comply may result in Forfeiture of Bond.
