Subsequently contacted by the operator later in the day to report the crew had completed the site cleanup. And stone was placed at the well head to reduce the amount of mud when it rained.

| Received a phone call from Don Collier (Fishburn service rig operator) that the Philp #1 (Trenton well) was flowing out of control. They where in the process of striping the tubing out (14 joints) and the well unloaded through the 4.5" casing. The well has not produced in two years and had no pressure on it at the start of operations. Oil and gas blew in the air (from the casing) and from the flow line into a lined earthen pit. The power tongs acted as a diverter and prevented the flow from reaching the crown of the service rig. They shut off all motors and evacuated the site (no one was injured) called Fishburn to send a water truck, vacuum truck and a backhoe. They notified state inspectors Tom Benko and Eric Daugstrup who responded. No emergency personnel where notified or showed up. After 45 minutes the flow slowed enough for them to safely put a swedge in the well head. The site impact is minimal and was contained to the pit (which they emptied), and light spray of oil on the ground behind the pump jack. The pump jack was thoroughly coated with oil. The back hoe operator expanded the flow pit to accept more volume. They are getting a new well head to hook up and resume workover.  Site OK. Inspector Benko has a video shot by the operator of the uncontrolled well flow.

1341857917 2012-07-09 Received a phone call from Don Collier (Fishburn service rig operator) that the Philp #1 (Trenton well) was flowing out of control. They where in the process of striping the tubing out (14 joints) and the well unloaded through the 4.5 casing. The well has not produced in two years and had no pressure on it at the start of operations. Oil and gas blew in the air (from the casing) and from the flow line into a lined earthen pit. The power tongs acted as a diverter and prevented the flow from reaching the crown of the service rig. They shut off all motors and evacuated the site (no one was injured) called Fishburn to send a water truck, vacuum truck and a backhoe. They notified state inspectors Tom Benko and Eric Daugstrup who responded. No emergency personnel where notified or showed up. After 45 minutes the flow slowed enough for them to safely put a swedge in the well head. The site impact is minimal and was contained to the pit (which they emptied), and light spray of oil on the ground behind the pump jack. The pump jack was thoroughly coated with oil. The back hoe operator expanded the flow pit to accept more volume. They are getting a new well head to hook up and resume workover. Site OK. Inspector Benko has a video shot by the operator of the uncontrolled well flow.

Identified that the operator was servicing the well and the site would be remediated on July 17. 2012. Contacted by The Communications office that there was a complaint from The USEPA; identified that aware of the situation.

Well has new well head with stripping rubbers, (changed out during well service in July). June 25, 2012: Photograph taken by trespassers shows old well head with paraffin accumulation, observed on facebook on 8/10/2012. On July 9, 2012 Contacted by Fishburn services that the well blewout while pulling tubing. All oil contained in lined pit and shut-in when inspectors arrived. Inspection reports completed. July 10th to 12th well was serviced, tubing and rods steam cleaned, paraffin accumulated on ground, used hay bails to control flow into lined earthen pit. July 14, 2012 trespassers took photo of paraffin on location and posted to eyesondrilling website. July 16, 2012 contacted by Com Center, aware of well service, Contacted regarding email of eyesondrilling. Contacted operator and Inspection reports completed. July 17, 2012 Operator reported cleanup of location is completed. July 18, 2012 Inspected location, site was cleaned up to satisfaction of Inspector. Photos collected and inspection report completed. August 10, 2012 re-inspect location as a result of photos taken of well head by trespasser. Collect photos of well head. Prepared this report.
