| I was on location with ODNR inspector Pat Shreve to try and determine what is in the hole and what will need done to properly plug this well when the permit to plug and abandon is used and a plugging rigs comes on location. We determined that there is likely casing on hangers with packing in the hole, likely being 2' and 3' inside. It was also agreed upon that a full cement plug would need to be cemented in this well to properly plug the well. Information from offset wells was also collected at this time to try and determine what could possibly in the well when it comes time to plug. Each one of the 10 wells on the property could have different down hole properties and each well will need to be addressed separately.

| We had a fishing tool on location to pull the broken 2" casing in the hole. So far 191' of 2" tubing has been pulled along with 932'' of 1" tubing and a gas escape packer set at 932' were also pulled. 33 steel and 21 wooden rods have been pulled as well. In the afternoon the 2' was pulled to 1341' and the well was cemented with 150 sacks of cement.

| Cement was tagged at 127'. A bond log and collar locator were run and found that the 6 5/8" casing was mudded in. The casing was attempted to be backed off but it could not and the well was cemented to surface.

| I was on location to see if any work has been done on this well that has been excavated out so that access to the well can be had. Currently lines from the well are running to a 1000L holding tank in case the wells produce any fluid. No changes to the wells have been completed at this time.
