| Associated with complaint # 4357. Well equipped with pumping unit with electric motor. A seep had occurred 50' down hill of the tank battery and scalded and area roughly 18 x 5'. The water in the seep was field tested and found to have 5664 ppm Chlorides. There was no surface evidence of contamination from the dike to the affected site. The Chlorides appeared to have moved under the soil. All contaminated soils shall be removed and disposed of properly. Any affected area shall be reseeded and mulched and steps shall be taken to encourage vegetative cover at the site.

| Josh Garland asked me to inspect the tank battery and potential brine contamination leaching from the 100-bbl steel tank migrating and discharging at the surface slope below the battery.  Further down the access road and at the same or higher elevation is the farmhouse and water well.  The landowner claims that the brine leachate is contaminating their drinking water supply from the well.  The landowners admitted that during certain times of the year, when the weather is dryer than usual, they have 2400 gallons of water delivered and poured into their water well to supplement their water supply.  I offered a few suggestions to Josh on what the operator could do to mitigate this problem without doing more aggressive remediation. Apparently there is also water analyses on the well water and the brine discharge water but I have not examined it.

| I met with Keith North of Keito to discuss clean up at the tank battery. The dike around the tanks contain contaminated soils and fluids, and after review it was decided the contaminated soil poses the threat of causing future contamination.  Keith will be removing the contaminated soils in the dike and replacing the lost soil.  I suggested to Keith moving the tanks may be prudent and we discussed options regarding placement of the tanks. Keith informed me the area downhill of the original contamination that was unable to support vegetation never originally got mulched. Keith had reseeded and mulched the area since then and a few small grass seedlings were taking hold today.

| Follow up to 1380727475. Associated with complaint # 4487. 2-4 inches of snow cover on ground, no immediate signs of surface contamination available. Quan-tab field test strip used to check for presence of Chlorides near area of concern. High range test strips indicated Chloride levels well below 296 ppm, and the Low range test strip around 30 ppm. The area does not appear to have been reclaimed and the tank battery is still in its original place at the top of the hill.

| The area associated with Compliance Notice  -758707775 appears to be leaching brine once again. The company performed remedial work and the violation was resolved, but I checked on the area due to an inspection request associated with a separate complaint on the same property. I was informed by one of the residents the area began to show signs of contamination again a 1-2 weeks after I had written off the violation. The contaminated area appears to be roughly the same shape and size as before and is in the same location as the first seep. Pictures and GPS points were taken. The soil was field tested with a high range Quan tab and found to have 783 ppm Chloride content. A low range Quan tab registered more than 607 ppm Chloride content. The 2" steel line running into the separator has significant corrosion but does not appear to be leaking.

Inspection Data

INSPECTION ID DATE DESCRIPTION COMMENT VIOLATION ID VIOLATION CODE VIOLATION COMMENT
-1171600054 2006-10-13 The dike still requires clean-up.
1372773232 2013-07-02 Associated with complaint # 4357 and inspection -758707775. A large area roughly60 x 75' has been excavated. The contaminated soils have been removed and the soil has been treated with Lime, seeded, and mulched. No negative impact to the nearby garden. No violations at the well head.
1380727475 2013-10-02 Well equipped with a pumping unit with electirc motor. Valves on the production cracked and valves on the surface casing open. Identification is faded and worn. The area downhill from the tank battery associated with complaint # 4357 and inspection -758707775 appears to still have a small scalded area of 5 x 10 feet. All producing leases shall be legibly identified in a conspicuous place on or near the well-head or the storage tank as to owner, lease name, well number, permit number where available, county, and an emergency telephone number.
1383150397 2013-10-30 Accompanied by Morgan Button. Reviewed site of contamination at tanks with well 34.009.2.1593 to discuss possible geological influences on shallow subsurface fluid movement of brine and ground water characteristics. The elevation of the dike was noted to be 945', the elevation of the seep was noted to be 927', and the elevation of the nearby water well was noted to be 933' with a fluid depth of 20-30 feet down.
1383229876 2013-10-30 Josh Garland asked me to inspect the tank battery and potential brine contamination leaching from the 100-bbl steel tank migrating and discharging at the surface slope below the battery.
1383924323 2013-11-08 I met with Keith North of Keito to discuss clean up at the tank battery.
1397066544 2014-04-09 Follow up to 1380727475. Associated with complaint # 4487. The identification tag has been placed at the tank battery. The contaminated soils have been removed from the dike, replaced with fresh soil, seeded and mulched. The small contaminated area downhill has grass growing in it.
139772710 2006-09-21 Oil puddles inside tank battery dike. Apparently a water tank overflowed. Found no offsite impact. Company representative in process of clean-up.
-2093057375 2006-10-19 There still was clean up needed inside the tank battery dike.
210612039 2014-07-03 Well equipped with pumping unit and electric motor. The valves on the tubing and the production casing were open.
273147946 2013-10-23 Follow up to 1380727475. Associated with complaint # 4487. The identification has been placed at the well head.
537154796 2014-02-11 Follow up to 1380727475.
632507001 2006-11-14 Clean up was completed and satisfactory.
-758707775 2013-06-19 Associated with complaint # 4357.
-793459654 2013-06-24 Associated with complaint # 4357 and inspection -758707775. No work has been done as of yet to remediate the seep.
