| My inspection this day to update I/O file.  New matrix score is 135, photos taken and this inspection added.  Project consists of 2 80+ bbl tanks sitting right off shoulder of township road 500' apart.  The north tank sits at the entrance of the barely existing lease road - drop off is sharp and steep with well located almost at creek level.  Top of casing is 2-3' above ground level with very old wellhead on 6.25 OD casing.  Wellhead is open to the air and when rock is dropped, find fluid level very far downhole.  Just above the well sits gumpy circle of oil where old tank has set - rotted completely away but should remove this contaminated soil to prevent runoff into creek.  Just below the well is spot about 10 x 10' full of water - may not have anything to do with well.  Local experts say this well is Peaker @750' deep.

| My inspection this day to check well after the flooding rains to see of oil overflowed well or otherwise got on ground.  I found no indication that oil had spilled from the well - undoubtedly, the well had been under water.

| I inspected this well today with Chuck McCrachen, Health Physicist for DOGRM to observe the old tank bottom located near the well.  Paraffin seems to be the main component of the oily sludge which is typical of old orphan wells involving clean-up.  We also inspected the well, still open to the atmosphere and giving off a slight oil odor, as well as the open pit below the well.  The new access has been marked by WNF and the GPS entered into this report.  The 2 abandoned tanks up along the township road were inspected, one being empty and the one closer to the access and containing a tank bottom.
