|  Preliminary Restoration status check inspection found the well site meets ORC 1509.072 with the area affected by the well pad construction restored and vegetation established. The well pad perimeter is fenced with a 8 chain link fence and signage posted at the well pad entrance as well as at each well head and at the tank battery. Emergency shut down devices are located at the well pad entrance the tank battery and Gas processing units and also inside the remote transmission unit building, and finally at the well head. On location a total of (8) wells are present with all wells drilled- stimulated and completed with the production valves in the open position at the well head. Also present on the well heads is annular pressure monitoring gauges with 250 pounds pressure showing between the 13.375 and 9.625 and 500 psi between the 5.5 and 9.625.  The company is monitoring and addressing any annular pressure. On the South side of the well pad is the production facility consisting of a total of (4) steel construction production tanks with a total of 400 barrels fluid capacity each. On the East side of the well pad is the production filtration and separation equipment consisting of a total of (8) gas processing units and (2 )dehydration units. Directly on the North East side of the well pad is the gas distribution point where the gas product is transferred over to Eureka Midstream.

| The purpose of my inspection is to witness the cementing of the 18.625 inch diameter Surface/Mine casing string. HP 531 Drilling rig could not get the casing passed 590 feet in depth. The casing was removed from the borehole and they will trip a 24 inch bit back into the borehole to clean the hole out. The cement job will not be able to take place at this time.

| The purpose of my inspection today is to witness the surface-mine string cement job. Mine void encountered from 787 feet through to 792 feet. HP 531 Drilling rig drilled a 24 inch diameter borehole to a depth of 910 feet. The 18.625 inch diameter casing was run to a depth of 874.4 feet. Allied Cement Services is on location to cement this casing string in two stages with a packer set at 667 feet. 310 barrels of fresh water and 20 barrels of gel were pumped ahead of the cement. 66.8 barrels of Class A cement with and 40 barrels water, 40 barrels of gel, and 160.7 barrels of fresh water were pumped for displacement to complete the first stage. The plug landed and the packer was energized. The second stage was completed using 234.9 barrels of Class A cement. 204.2 barrels of displacement were pumped and 44 barrels of cement returns were witnessed.

| The purpose of my inspection today is to witness the 120 feet deep 30 inch diameter conductor being cemented into a 36 inch diameter hole. No water was encountered during the drilling process. Stingray Cementing is on location to complete the cement operation. During my inspection Stingray pumped 15 barrels of gel, 10 barrels of water ahead of 110 barrels of Class A cement at 15.6 pounds per gallon and displaced with 87 barrels of water.  30 barrels of cement returns were achieved.

0548069865 2017-08-31 Preliminary Restoration No Violations Preliminary Restoration status check inspection found the well site meets ORC 1509.072 with the area affected by the well pad construction restored and vegetation established. The well pad perimeter is fenced with a 8 chain link fence and signage posted at the well pad entrance as well as at each well head and at the tank battery. Emergency shut down devices are located at the well pad entrance the tank battery and Gas processing units and also inside the remote transmission unit building, and finally at the well head. On location a total of (8) wells are present with all wells drilled- stimulated and completed with the production valves in the open position at the well head. Also present on the well heads is annular pressure monitoring gauges with 250 pounds pressure showing between the 13.375 and 9.625 and 500 psi between the 5.5 and 9.625. The company is monitoring and addressing any annular pressure. On the South side of the well pad is the production facility consisting of a total of (4) steel construction production tanks with a total of 400 barrels fluid capacity each. On the East side of the well pad is the production filtration and separation equipment consisting of a total of (8) gas processing units and (2 )dehydration units. Directly on the North East side of the well pad is the gas distribution point where the gas product is transferred over to Eureka Midstream.
1204432995 2016-11-26 Casing String Witnessed No Violations The purpose of my inspection is to witness the cementing of the 18.625 inch diameter Surface/Mine casing string. HP 531 Drilling rig could not get the casing passed 590 feet in depth. The casing was removed from the borehole and they will trip a 24 inch bit back into the borehole to clean the hole out. The cement job will not be able to take place at this time.
1480352321 2016-11-27 Casing String Witnessed No Violations The purpose of my inspection today is to witness the surface-mine string cement job. Mine void encountered from 787 feet through to 792 feet. HP 531 Drilling rig drilled a 24 inch diameter borehole to a depth of 910 feet. The 18.625 inch diameter casing was run to a depth of 874.4 feet. Allied Cement Services is on location to cement this casing string in two stages with a packer set at 667 feet. 310 barrels of fresh water and 20 barrels of gel were pumped ahead of the cement. 66.8 barrels of Class A cement with and 40 barrels water, 40 barrels of gel, and 160.7 barrels of fresh water were pumped for displacement to complete the first stage. The plug landed and the packer was energized. The second stage was completed using 234.9 barrels of Class A cement. 204.2 barrels of displacement were pumped and 44 barrels of cement returns were witnessed.
1481641308 2016-12-13 Drill / Deepen / Reopen No Violations During my inspection Drilling Rig HP 531 was tripping out of hole with the drill pipe after reaching a total depth of 17,180 feet in the lateral.
1486121629 2016-11-07 Casing String Witnessed No Violations The purpose of my inspection today is to witness the 120 feet deep 30 inch diameter conductor being cemented into a 36 inch diameter hole. No water was encountered during the drilling process. Stingray Cementing is on location to complete the cement operation. During my inspection Stingray pumped 15 barrels of gel, 10 barrels of water ahead of 110 barrels of Class A cement at 15.6 pounds per gallon and displaced with 87 barrels of water. 30 barrels of cement returns were achieved.
