| On the day of my inspection I was accompanied by Deputy Chief Bob Worstall and Inspector John Fleming.  This pad contains four properly labeled producing wellheads. Production equipment includes twelve 400 barrel steel tanks in their own polymer lined steel containment, one 50 barrel steel tank, four GPUs, four sand seperators, and a combustor. Surface annular pressure is 0 and intermediate annular pressure is 0. Production records are current in RBDMS.

| I am on site today to conduct a remedial cement job on the 10-3/4 inch diameter casing. On the last job - 03/13/2013 Yost Drilling Rig # 3 set 1724 ft of casing and Stingray Cement Service c/w 1080 sks of Class A cement. The cement job was not complete because the cement could not be circulated to surface. The cement was drilled out and a cement bond log found the top of the cement in the annular space at 980 ft. 1-1/4 inch diameter tubing was installed t to 960 ft and Stingray Cementing Service  circulated cement to surface. The number of sacks used to fill the annular space was  680  sks.

| I am on site today to witness the running and the cementing of the 10-3/4 inch diameter casing string. Yost Drilling Rig # 3 drilled a 14-3/4 inch diameter hole to 1730 ft and set 1724 ft of casing. Stingray Cementing Service c/w 1080 sks of class A - 15.6 lb cement. The casing is  being cemented without a cement head and the swedge will need removed and a wiper plug inserted. After that was done Stingray could not get any circualation because the cement had set up. Yost Drilling did try to move casing up and down and found out it has been cemented in place. Next they will need to drill out the cement and log the well. After the find out the top of the cement in the annulus tubing will be installed to fill it back to surface.

| I am on site today to check on the drilling status of Yost Drilling # 7  During my inspection I discovered that they have drilled to 6103 ft and are now stuck in the hole. They have backed off one joint above the drill collars at 5785 ft. The drill  collars and the 9-7/8 inch diameter drilling bit remain in the well bore. The plan is to run in with a fish grapple and grab the drill pipe. Then while connected with tension run inside and perf holes in the drill collars and try and establish circulation. They will have to jar loose almost 100 ft of fill on the back side. I am worried that the depth and weight for that rig will brake the drilling line when the jars go off multiples of times. The rig is next to two high pressure wells which are in production. If the drilling line comes apart and the blocks fall down it could impact the two wells. I contacted Chad Shoffner and advised him of my concerns. Chad said that he would contact their office and get the approval to shut the wells  in while the jars are in use.

Inspection Data

Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
0019359868 2013-03-14 Drill / Deepen / Reopen No Violations I am on site today to see if the well has been drilled out and a cement bond log completed to locate the top of the cement on the back side of the casing. During my inspection I found that they are at 1000 ft drilling out cement.
0226106071 2018-12-13 Production Wells No Violations On the day of my inspection I was accompanied by Deputy Chief Bob Worstall and Inspector John Fleming.
1193883210 2013-12-10 Preliminary Restoration No Violations On the day of my inspection the site passed preliminary restoration according to 1509.072A.
1322042829 2013-03-15 Drill / Deepen / Reopen No Violations I am on site today to conduct a remedial cement job on the 10-3/4 inch diameter casing.
1363171489 2013-03-13 Drill / Deepen / Reopen No Violations I am on site today to witness the running and the cementing of the 10-3/4 inch diameter casing string.
1365612163 2013-04-10 Drill / Deepen / Reopen No Violations I am on site today to check on the drilling status of Yost Drilling # 7 During my inspection I discovered that they have drilled to 6103 ft and are now stuck in the hole.
1541356625 2013-09-18 Production Wells No Violations The well is set up to frac. I am here to check on the containment and did not find any leaks at this time.
