| I was on location to perform a status check inspection.  I found during my inspection the well equipped with a plunger lift unit that was in production.  The tank battery consisted of a 210 barrel production tank, a 50 barrel, poly brine tank and a separator.  A dike surrounded the tank battery.  Identification signs were posted at the well and tank battery.  There was a wooden fence around the production equipment and a locked, chain link fence around the well.  The door to the wooden fence was lying on the ground.  A bull plug was installed on the load line.  There was no urban safety equipment on the production tank.  The lease road was in good condition.  I called PAC Drilling and instructed Gus to:

| I conducted a compliance notice follow-up inspection for compliance notice 1845806637, regarding multiple compliance issues related to urban rule requirements.  At the time of this inspection, the well fence man door still needed a lock.  The brine tank valve leak was not actively leaking, at the time of this inspection, but the tank did not appear to be repaired.  The required urban safety equipment was still missing from the production tank, and needs to be installed per 1501:9-9-05E(4)(b-e).  There was a wooden fence with no lock around the tank battery. Install a lock on the tank battery fence. The identification sign was missing the required information per 1501:9-9-05E(1)(a-e).  Landscaping at the tank battery has not been installed per 1501:9-9-05(E)(6)(a-c).

Install a lock on the tank battery fence. The identification sign was missing the required information per 1501:9-9--e).  There was no landscaping at the tank battery; however, the tank battery is located approximately 1,500 feet south of Route 304 on commercial/industrial property with trees and vegetation on the north and east sides.  The access road was stable with no gate at the entrance.  The access road was the entrance for several commercial/industrial businesses.  The tank battery is shared with permit 2-3874.

I found during my inspection the well equipped with a plunger lift unit that was in production. The tank battery consisted of a 210 barrel production tank, a 50 barrel, poly brine tank and a separator. A dike surrounded the tank battery. Identification signs were posted at the well and tank battery. There was a wooden fence around the production equipment and a locked, chain link fence around the well. The door to the wooden fence was lying on the ground. A bull plug was installed on the load line. There was no urban safety equipment on the production tank. The lease road was in good condition. I called PAC Drilling and instructed Gus to:

Install the required safety equipment by 9/26/2014 Repair the wooden door on the fencing surrounding the production equipment A follow-up inspection will be performed by 9/29/2014.

0079554623 2019-02-04 UrbanProduction Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 1845806637, regarding multiple compliance issues related to urban rule requirements. At the time of this inspection, the well fence man door still needed a lock. The brine tank valve leak was not actively leaking, at the time of this inspection, but the tank did not appear to be repaired. The required urban safety equipment was still missing from the production tank, and needs to be installed per 1501:9-9-05E(4)(b-e). There was a wooden fence with no lock around the tank battery. Install a lock on the tank battery fence. The identification sign was missing the required information per 1501:9-9-05E(1)(a-e). Landscaping at the tank battery has not been installed per 1501:9-9-05(E)(6)(a-c).

PAC needs to: 1) Install locks on the well and tank battery man doors. 2) Post an identification sign to be in compliance with 1501:9-9-05E(1)(a-e) at the tank battery with: Well owners address and phone number Property street address or nearest address to the access road entrance. 3) Install urban safety equipment on the production tank as required by 1501:9-9-05E(4)(b-e): Functioning low-pressure relief valve. Tank hatch lids shall have a functioning seal and the hatch shall be secured at all times. Functioning lightning arrestor 4) Install the landscaping at the tank battery location as required per 1501:9-9-05(E)(6)(a-c).

Landscaping at the tank battery has not been installed. The tank battery is shared with permit 2-3874. I contacted Rusty Hornhill of PAC Drilling, LLC about addressing the leak on the pick-up line and cleaning up any impacted soil from the brine leak.

1846806637 2018-05-11 UrbanProduction Wells Violations Noted At the time of this inspection, the well was equipped with a plunger lift assembly. The annulus and flow line valves were open. There was an identification posted on the chain link fence that surrounds the well with no lock. A lock needs installed at the well fence. The tank battery has two storage tanks and a separator. The dike has been rebuilt on the north end of the tank battery. The pick-up line has a plug, but a missing lock. The required urban safety equipment was missing from the production tank, and needs to be installed per 1501:9-9--e). There was a wooden fence with no lock around the tank battery. Install a lock on the tank battery fence. The identification sign was missing the required information per 1501:9-9--e). There was no landscaping at the tank battery; however, the tank battery is located approximately 1,500 feet south of Route 304 on commercial/industrial property with trees and vegetation on the north and east sides. The access road was stable with no gate at the entrance. The access road was the entrance for several commercial/industrial businesses. The tank battery is shared with permit 2-3874.
