The bottom formation plug had been set on the preceding day. At the time of this inspection, I inspected the wellhead and found no measurable residual pressure. Crews from Appalachian Well Surveys were on location to shoot the production casing. The remaining plugging activities were executed and completed according to plan and without incident or problem. I conducted continuous ambient air monitoring for the presence of explosive gas and hydrogen sulfide and observed no readings. Upon completion, the top surface plug was stable at grade. Crews cut the well casing to 30 inches below grade as prescribed under 1301:9-11. The former tank battery had been dismantled and the site graded level. |

The wellhead and surrounding area are free of signs of leaks or other impacts. Access to the wellhead and battery are via the concrete-paved entry to the residence, and then a short continuation (150) to the west. . The battery is comprised of 1 steel 100-barrel production tank, 1 50-barrel steel brine tank, and separator unit. Spacing from well to battery, and spacing of elements within the battery meet regulations. The load-out is bull-plugged. The secondary containment at the battery appears well maintained. Process safety equipment is present. I did not observe any signs or evidence of leakage, spills, or problems. The ID placards at the wellhead and battery are in good condition. |
