1396286137 2014-03-31 UrbanProduction Wells No Violations The producing unit is comprised of a wellhead with mechanical plunger lift in a chainlink enclosure. The enclosure is locked and has a compliant ID placard. The 8.625-inch casing is exposed and valved. The tank battery is located adjacent to the well, in a separate locked chain-link enclosure. The battery is comprised of 1 100-barrel steel production tank and 1 50-barrel poly brine tank, and a separator unit. The internal spacing, and spacing to well are compliant. There is secondary containment, complaint ID, and process safety equipment. There is a Dominion metering run outside the battery. There are no indications of leaks or spills. The lease road is in good condition and is draining well. There is a locked gated entry at the lease road heads with a placard.

1726730248 2017-11-20 UrbanProduction Wells No Violations I inspected this well as part of routine inspection activity. The well and associated tank battery, are located on an otherwise undeveloped parcel. At the time of this inspection, fittings and connections at the well and battery were secure and free from leakage or problems. The load-line valves were secure and locked. Tank lids were closed and secure. Secondary containment as intact and free from signs of release, leaks, or problems. The well was in active production. Production reports were current. Bonding and insurance were active and current. I walked and inspected the areas surrounding the wellhead and battery and found no evidence of impacts from leaks, releases, or degradation to the natural greenscape.

1906630998 2015-12-16 UrbanProduction Wells No Violations I inspected this producing well as part of routine inspection activity. The well and associated battery are located in a densely wooded area on the west side of County Line Road. At the time of this inspection, the areas surrounding the wellhead and battery were comprised of deciduous tress and meadow grass. The topography slopes to the west, towards the Chagrin River Valley. At the time of this inspection, the wellhead and tank battery chain-link enclosures were locked and secure. ID signage was present at the head of the lease road, and at both the wellhead and battery locations. The lease road gate was locked and secure. Fittings and connections at the wellhead were secure. Backside pressure at the 8.626-inch casing was showing zero. Fittings and connections at the battery were secure. The secondary containment was present and intact. The interior areas were dry. Tank hatch lids were sealed and secure. Spark arrestor element, carbon filter, and low-pressure relief valve is present. The load-lines were bull-plugged and secure but require locks. I walked the surrounding areas and areas down-gradient of the producing elements and did not see evidence of impact or drainage issues relating to the well operation. The gravel-paved access road was dry with no marked erosional deficits or problems.
