At the time of this inspection, I observed the following: the were no locks at the load-line valves and the 210-barrel tank remains in place, but was not being actively produced into. The ID signage remained from prior ownership and not accurate. I had previously contacted the company on several occasions since the issuance of the compliance notice regarding the deficiencies. The company had made pledges to remedy the deficiencies.

0641896138 2017-05-05 UrbanProduction Wells No Violations I inspected this well location as a follow-up to a previously-issued compliance notice for the following: missing locks at the load-line valves. At the time of this inspection, I observed the following: The wellhead was comprised of a plunger-lift system with associated tank battery (shared with the #2-1188) well. The load-line valves were not secured with locks. In addition, the ID signage has not been upgraded to reflect the change in ownership and is missing information required under OAC 1501:9-9-05(E)(1). I have contacted John Magnussen about the unaddressed deficiencies and he advised that the compliance issues are being tended to as soon as possible. I also attempted to contact Richard Osborne Sr. and, as of the filing of this report, he has not responded to multiple messages.
0733548168 2015-04-22 UrbanProduction Wells No Violations I inspected this producing unit as part of routine inspection activity. The well and battery are located in a open, grassy portion on a open, undeveloped area within a newer residential subdivision. The well is comprised of a mechanical plunger-lift system in a board-on-board enclosure which is locked. Fittings and connections at the wellhead are secure. The battery is comprised of one 210-barrel steel production tank and one 100-barrel brine tank and separator unit in a separate board-on-board enclosure. Fittings are secure however I observed a recent release of oil within the diked area which has not been addressed. There is approximately 1 barrel of oil floating on water within the battery and staining the containment ballast. I inspected the areas outside the dike and observed some oil staining to the grass on the north side of the dike, but no free oil beyond the dike. I also walked to the creek which lie at the bottom of the ravine adjacent to the well and did not observe any oil. I contacted John Magnusson with John D Oil and advised him about the oil in the dike. He indicated no knowledge of a release and would be sending a crew ASAP.
0823819888 2017-12-05 UrbanProduction Wells Violations Noted I inspected this well location as a follow-up to a previously-issued compliance notice issued for unsecured load-line locks. At the time of this inspection, I observed the following additional deficiency: the 210-barrel steel stock tank, which is shared with the #2-1188 (Greg 13) was observed to have serious and substantial corrosion of the sidewalls resulting in failure of the tank integrity. The tank must be taken out of service immediately, and removed from the production system within 30 days. At the time of this inspection, I observed the following: there were no locks at the load-line valve or on the mechanical separator enclosure. In addition, the ID signage has not been updated to reflect the change of ownership that occurred over 12 months prior to this inspection. I have contacted the company on several occasions since the issuance of the compliance notice. The company has reported that problems with personnel and/or lack of funds, have prevented them from being in compliance.

Repeated calls to the owner have not produced any demonstrable action or results.

The unit ID is present and meets requirements. The load-outs are locked. The battery secondary containment is good. The battery is shared with the Greg Jr. unit (#21188). Saftey equipment is present in accordance with 1501:9-9-05E. No problems or violations noted.
1430489439 2015-05-01 UrbanProduction Wells Violations Noted I inspected this producing unit as a follow-up to an earlier inspection on 04/22/2015 whereby I observed a release of 0.5 to 1 barrel of crude oil into the secondary containment of the tank battery. At the time of that inspection I immediately notified John Magnuson with Great Plains Exploration who indicated the issue would be addressed ASAP. As of this date, acted John Magnuson and have advised that clean-up must include removal of at the majority of the oil released remains along with all of the oil-impacted dike gravel-ballast and all free oil and oil-impacted dike ballast and berm material. As a follow-up I was contacted by Matt Lutta with Great Plains regarding the incident. Matt and his crew arrived at the location at 1150 AM to begin mitigation and clean-up. I advised Matt that clean-up will not be considered complete until all residual oil and oil-impacted material has been removed or cleaned to the maximum extent practicable. These activities should be completed within 30 days or be subject to additional enforcement action.
1442339968 2015-09-15 UrbanProduction Wells No Violations I inspected this well as a follow-up to a previously-issued Compliance Notice relating to a release of oil which had occurred within the confines of the secondary containment. At the time of the incident, approximately 1 barrel of oil had been released into the containment from an over-fill situation. At my original inspection, I did not observe an oil or oil-related impact outside the containment for the tank battery. At the time of this inspection, the oil has been recovered and the interior of the containment has been restored. Actions necessary to complete the remedy are complete. The lease road gate is lacking signage and Ive contacted the owner to remedy.
1752317045 2018-12-03 UrbanProduction Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 823819888, regarding the removal of the 100 BBL tank due to loss of integrity. The well was a plunger lift assembly and not in production at the time of this inspection. The well was fenced with a locked man door. The tank battery consisted of one 210 BBL tank, one 100 BBL tank and two mechanical separators. The fence door was broken and unlocked on the separator fence. The diked tank battery was shared with well 2-1188 and did not have a locked man door. The 100 BBL tank has not been removed from the tank battery and the load lines and tank valves have not been locked. The identification for this well needs to be updated to reflect current ownership information. The lease access was stable and gate at access locked.

At the time of this inspection, I observed the following: a wellhead with mechanical plunger lift system in a board-on-board enclosure. Fittings and connections at the wellhead were secure. The battery is comprised of one 210-barrel steel production tank, one 100-barrel steel brine tank and separator in separate board-on-board enclosures. Enclosure gates were locked and secure. Hatches were closed and sealed. Secondary containment was present and intact. Fittings and connections were secure and free from signs of leakage or problems. I walked the areas surround both the well and battery and did not observe any signs of impact relative to the production activity. Signage was present. I noted the following safety issues: The load-line valves were not locked. The owner must remedy the following by 04/04/2016: secure the load-line valves with locks.
