Signage is posted at the well pad entrance. Halliburton crews are on site preparing to begin the well stimulation 5.5 diameter production casing toe preparations.
0296998331 2016-12-29 Drill / Deepen / Reopen No Violations On the day of my inspection Frontier Drilling Rig #32 had completed the walk to the well from the 34121246120000 Wehr 2H and were nippling up the blow out preventer with Ricon Services onsite to complete testing. I met with Antero Drilling Consultant Brandon Jeffcoat and Drilling Superintendent Joe Canady and completed walk around.
0327277029 2017-04-11 Completion Testing No Violations The purpose of my inspection today is a status check on a hydraulic fracturing operations. During my inspection Antero Company men, Allied wireline, and US Well Service were on location. I witnessed US Well Service rigging up to preform hydraulic fracturing on the 18th stage. Pad inspection found no violations present.

Production valves were in the open position. The production equipment consisted of twelve 400 barrel tanks within steel line containment, six sand traps, and six gas production units. The sand traps were flowing back into a 400 barrel gas buster tank. The pipeline tie in was located on the west side of the pad. The identification was located at the wellhead. Two Myers trucks were hauling produced fluids from the tank battery and one Antero Company man was on location.

Also on location is QES Pressure Control (AKA Great White) providing snubbing unit services. The well head assembly consists of a Cameron sub base and (B) Section main lower well head assembly. Sitting atop the lower section is a (BOP) blow out prevention consist owned by Bass Energy consisting of a manual master valve-
1473345763 2016-09-08 Casing String Witnessed No Violations I arrived on location to witness Express Energy Services set conductor casing on the Wehr 1H. Prior to my arrival Inspector Conrad had approved them to set the conductor to 80 feet. We set the conductor using 257 sacks of class A cement with 2% NaCl and about a half a pound of flake. The weight of the cement was 15.6 pounds and had a 1.18 yield with a 32.19 mix. We used a total of 43.35 barrels of fluid for displacement. When we finished setting the casing we had a 10 foot shoe. We lost about 15 barrels of cement on the surface. After we had finished setting the conductor we had about 15 barrels of cement come back to surface and no water.
1475661521 2016-10-05 Casing String Witnessed No Violations 10/05/2016- Frontier Drilling rig # 32 is now on the hole and has drilled a 17.5 surface borehole to 1,089 and ran 1,071 of 13.375 54.5 lb/ft J-55 casing with a guide shoe and float collar casing equipment ran. C&J Cement service cemented the casing using 1009 sacks of class A-2% calcium .25 lb/sk lcm with circulation established prior to cementing and 25 barrels of 6% gel spacer ran ahead of the cement, upon the landing of the cement plug the casing was pressure tested to 500 psi over displacement pressure with good cement returns witnessed to surface inside the cellar.

1483714309 2017-01-06 Drill / Deepen / Reopen No Violations On the day of my inspection Frontier Drilling Rig #32 was drilling their lateral with an 8.75 inch diameter bit at 16172 feet on 11.6 pound per gallon (ppg) mud simulating 13.6 ppg. Frontier Drilling Rig #32 is planning on drilling to projected total depth of 16812 feet. I completed walk around with Antero Consultant Steve Guffey revealing proper containment was in place.
1498153866 2017-06-22 Completion Testing No Violations Status check inspection on the well completion process found a service rig on the hole owned by Fortis Energy. The rig has completed the completion clean out run and drilled out all the completion plugs to the well bore total depth inside the 5.5 diameter production casing. The rig is pulling the drill string keeping the well bore loaded with fluid and static while pulling the string to avoid any well bore gas migration. Upon recovering the 2.875 diameter workover string the production string will be inserted into the well bore.
1504943869 2017-04-17 Completion Testing No Violations US Well Services, Allied Horizontal Wireline, and Antero Company men were all on location. During my inspection accompanied by ODNR Regional Manager Brent Bear we verified that the zipper hydraulic fracturing operations were temporarily put on hold due to slightly high magnitude readings detected at 12:00 PM 4/17/17. Operations were put on hold following setting wireline perforations operations on stage 28. Top perforations for that stage were at 11265 feet, bottom perforations were at 11433 feet, and the frac plug was placed at 11448 feet. A plan will be worked out for continuing operations between ODNR Seismic section and Antero Resources Completions Manager.

The well was equipped with a production tree style wellhead. Production valves were in the open position. Annular pressure monitoring was present and evaluated. The production equipment consisted of twelve 400-barrel tanks, six sand traps, and six gas production units. The pipeline tie in was located on the west side of the pad. The identification was located at the wellhead. Myers Trucking was hauling produced fluids from the tank battery.
