Well was a spud conductor. Conductor casing had a steel plate covering it at time of inspection. Cellar was empty and clean. No violations were noted at time of inspection.
2818943 Primary Facility 2018-12-26 Routine/Complete Inspection No Violations Noted A H&P rig is drilling on the Smith Well pad. The Department representative did not locate the rig manager so Department could not determine which well was being drilled.

The site had a primary containment overlaid with wood plank rig matting with beveled edges (see photo). All materials held on containment and within secondary containment. The overall site conditions were clean and well organized. No leaks or spills detected. No violations.

2822744 Primary Facility 2019-01-03 Routine/Complete Inspection No Violations Noted Weather was 38° and cloudy at time of inspection. Met with Inflection representatives Jamie Czerneski and Greg Malekos on location. Well was covered with a steel plate at time of inspection. This well recently had the surface 18 5/8" casing cemented after the rig skidded back to the 10H to begin drilling the first intermediate. Reviewed the job log from the cement job and no discrepancies were noted at time of inspection. Rig will move back to drill subsequent strings. No violations were noted at time of inspection.
2827926 Primary Facility 2019-01-15 Routine/Complete Inspection No Violations Noted Weather was 25° and cloudy at time of inspection. Met with Inflection representatives Ross Drjnevitch and Greg Malekos on location. Halliburton was on location rigging up to cement the 13 3/8" intermediate casing. No violations were noted at time of inspection.
2835463 Primary Facility 2019-02-05 Routine/Complete Inspection No Violations Noted Weather was 37° and sunny at time of inspection. Met with Inflection representatives Greg Malekos on location. Well recently had the first intermediate string of 13 3/8" casing drilled and cemented. Well was closed in at time of inspection with a nightcap. Water in the cellar was frozen solid at time of inspection. No violations were noted at time of inspection.
2840031 Primary Facility 2019-02-13 Routine/Complete Inspection No Violations Noted Weather was 30° and cloudy at time of inspection. Met with Inflection representative Jamie Czerneski on location. This well was shut in with a night cat at time of inspection. 0% combustible gas was detected in the cellar at time of inspection. Well is being batch drilled with 5 other wells on location. No violations were noted at time of inspection.
2840560 Primary Facility 2019-02-14 Routine/Complete Inspection No Violations Noted I met Ross Drnjevich of Inflection on the site. H&P 521 on location, drilling the 2HN well at 8300'. The chemical containment appeared to be in good condition, but also contained several inches of snow and ice. The secondary containment for the mud tanks had been bolted on with wood strips on the inside of the containment in the back, approximately 4-5" down from the top. Although it is unlikely that this brought the containment volume below 110% of the volume of the tanks, I still recommended to Ross that these liners be attached on the outside of the berms. Meltwater had built up to 3-4" deep on the backside of the pad, and Ross told me he would have it vacced.
2842015 Primary Facility 2019-02-20 Routine/Complete Inspection No Violations Noted Weather- 21 degrees and cloudy at time of inspection

Met with Inflection representative Ross Drjnevitch and Greg Malekos on location. Well was closed in with a nightcap at time of inspection. Wells on pad are being batch drilled. No violations noted at time of inspection.

2844545 Primary Facility 2019-02-26 Routine/Complete Inspection No Violations Noted Weather was 24 degrees and sunny at time of inspection. Met with Inflection representative Ross Drjnevitch on location. They are currently drilling their second intermediate to a depth of 8215'. They are estimating to TD Thursday morning and cement on Friday afternoon. No violations at time of inspection.
2849876 Primary Facility 2019-03-07 Routine/Complete Inspection No Violations Noted Weather was 12 degrees and sunny. Met with Inflection representative Jamie Czerneski and Greg Malekos on location.
