On site for a new spud inspection. Minichi Contracting Group was on site to drill and run the conductor pipe for the 1H, 2H, 3H, 4H, and 5H wells. I met with driller Ralph Dewald. The rig was on the 4H at a depth of 75'. The 2H was completed at a depth of 180'. The 3H was completed at a depth of 176'. The 5H was completed at a depth of 175'. They plan to drill the 1H when the 4H is completed. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2698094 Primary Facility 2018-02-26 Drilling/Alteration No Violations Noted At time of inspection.

Met with company man Robert Gray. Falcon 24 was on site to drill the top-hole section of the well. Proposed TD for the top-hole section is 5200'. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2733451 Primary Facility 2018-05-23 Drilling/Alteration No Violations Noted At time of inspection.

Met with company man Andrew Griffith. SDC 45 was on site and drilling the lateral section of the well. They were drilling the curve at time of inspection. Proposed TD for the well is 12,233'. There were water trucks vacuuming water from the site off the containment. Andrew said they noticed a sheen on the ground and believes it was from trucks coming onto the location. I took a walk around the location and there was no sheen noticed on the standing water on the rig containment, there were no obvious concerns noticed. There were no violations noted.

2751787 Primary Facility 2018-07-13 Routine/Complete Inspection No Violations Noted At time of inspection.

Met with company man Kyle Wilkins. US Well Service was on site to frac the 1H, 2H, 3H and 4H wells. They were on stage 19 out of 25 on the 1H and running a plug and perf in the 3H when I arrived. There were 15 stages completed out of 24 on the 2H. There were 18 stages completed out of 26 on the 3H. There were 15 stages completed out of 20 on the 4H. The water was being pumped to the location from the Zeffer production fluid impoundment and the Young fresh water impoundment. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.

2753552 Primary Facility 2018-07-16 Routine/Complete Inspection No Violations Noted I performed an RTNC inspection in conjunction with Jon Sassi at the Woosman well pad. The wells associated with this pad are the 115-21054, 115-22377, 115-22402, 115-22378, 115-22403, 115-22379. The well site was being fracked at the time of inspection, The company man was Mark McPherson.

The site had good growth and was solid and stable at the time of inspection.

There were no erosional issues noted with the site at the time of inspection.

2760408 Primary Facility 2018-07-20 Follow-up Inspection No Violations Noted The Department was on site to conduct a follow up investigation of a spill that occurred on 7/13/2018. Present representing the Department were Frank Hoehle (WQS) and myself. We arrived on site at 1215 hours.

We talked to Donnie Torres (Well Site Supervisor-SWN). He informed us that crews were setting up on site to perform drill out and flowback operations. The one producing well on the pad was shut in. The others have been fully drilled and stimulated. Drill out operations on the plugs had not yet began.

The release occurred on the southwest corner of the pad, when a blender and its associated piping failed, causing a release of production fluid and freshwater in containment. The estimated release volume is not known. Material in the immediate area under the containment was screening high for conductivity. The material was excavated, but more high-conductivity water kept infiltrating the sump area below the containment. At the time of the initial response inspection, it did not appear as if there were a

2785322 Primary Facility 2018-10-05 Routine/Complete Inspection No Violations Noted At time of inspection.

The well was recently completed and put in production. The well tag was on the well and correct. The cellar was set up with a plastic corrugated pipe around the well for inspection access. The cellar was full of water and there was no bubbling noticed. There were twenty 500bbl frac tanks on containment from a recent frac. Most of the tanks were empty and the containment appeared to be in good condition. I took a walk around the location and there were no obvious concerns noticed. There were no violations noted.
