The weather was 31 degrees and sunny. An adequate site ID sign was observed at the entrance to the access road. The E&S plan, the PPC and the onsite inspection log were located in the mailbox onsite. The receiving stream was listed as UNT to Crooked Creek, WWF. The total disturbed area was listed as 5.96 acres. The site was last inspected by company personnel on 11/19/2015 with the following comments “No new issues visible. Currently raining.” On 11/5/2015 the following comments were observed in the onsite inspection log, “1. Small crack in foundation above fill slope, center of pad. Monitoring. 2. STs holding water. 3. Minor erosion on fill slope appears unchanged. Monitoring.”

The weather was 31 degrees and sunny. An adequate site ID sign was observed at the entrance to the access road. The E&S plan, the PPC and the onsite inspection log were located in the mailbox onsite. The site was last inspected by company personnel on 10/22/2016 with the following comments “Erosion starting on front side of pad and in corners. Sed trap draining into neighbors back yard”

As of today’s inspection, the site appears to have been restored recently. ST1 and ST2 have been replaced by a large infiltration basin below the fill slope of the pad. Due to recent above average precipitation events multiple areas of erosion are visible on the sidewalls of the basin and off the corners of the pad. Please see attached photos. The weather was cloudy 30°, fresh snow accumulation of ~3". No activity was observed onsite. No puddles observed on the pad surface. No vegetation was observed, but the site appears to be stable. 2671834 Primary Facility 2017-12-15 Routine/Partial Inspection No Violations Noted Spoke with Heath McAfee (SWEPI OSR) on site. He said Ensign 162 started drilling the 1st of the new Utica wells (24H) on December 4, 2017. No electrical conductivity readings taken today, as the site is frozen.
2722259 Primary Facility 2018-04-24 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection inspected the Neal 815 Well Site located in Chatham Twp, Tioga Co. on 4/24/2018 @ 1300 hours. The weather was 61 degrees and partly sunny. Phil Tillitt, SWEPI OSR and Justin Midgley, SWEPI Engineer were present onsite for the inspection.

Shell is in the process of fracking 4 Utica wells on the pad, the 21H, 22H, 24H and 26H. The rack is scheduled to be complete in 4 to 5 days. Water was observed in the ditches surrounding the pad. Puddles on the pad surface were tested for conductivity and found to be in normal range. The Infiltration basins below the fill slope of the pad were holding water.

CTS#333786 can be closed out at this time.

No violations are noted at this time. I departed the site at 1415 hours. -ELN
