0061097964 2019-04-17 Production Wells No Violations On this day, I found the well equipped with 4.5 inch diameter casing and 1.9 inch diameter tubing with the production valves open. It is setup to be produced by means of a pumping unit with rods and a gas engine. The production facility consists of (3) 210-barrel steel tanks, (2) separators, and (2) orifice sales meters with current charts all contained within an earthen dike. The facility is shared with permit numbers 9739, 9740, and 9752. The tanks are properly identified and the production records are current.
0257889389 2014-04-02 Casing String Witnessed No Violations Witnessed the cementing of the production string. Good circulation was achieved throughtout the job. When the plug was landed they pressured it up to 1000 psi. It held for 5 minutes.

0770164223 2014-04-01 Casing String Witnessed No Violations Witnessed the cementing of the surface casing string. Good circulation was achieved and good returns were observed. After the plug landed casing string was pressured up to 500 psi. and it held for 5 min. The well burped after the plug landed and cement level fell out of sight. The back side of the surface casing was then topped off from the surface. Cement was brought to the surface and it stayed up. No chlorides were detected in the mix water. Accompanied by Inspector LJ Parkins.
1396381631 2014-04-01 Drill / Deepen / Reopen No Violations Accompanied Inspector Andy Thomas to witness the cementing of 300 of 7 surface casing. There was good circulation observed as well as good cement returns at the surface. The well released an air pocket during cementing and caused the cement to drop in the hole. Formation Cementing toppped off the hole with cement from the top of the casing.
1505930405 2017-09-20 Preliminary Restoration No Violations The inspection this day revealed that this well is equipped with 8.625 inch surface casing, 4.5 inch production casing, 1.5 inch tubbing, rods, and a pumping unit. The valves on the wellhead are open at the time of inspection. The tank battery is equipped with three (3) 210 bbl. tanks, two (2) vertical separators, and two (2) meters with current charts in them. All production equipment will be shared between 29752, 29739, 29740, and 29738. All drilling equipment has been removed from site, pits have been closed, and the area has vegetation growing on it. There are no issues to report and preliminary restoration has been passed.
1671996319 2016-06-09 Preliminary Restoration No Violations The purpose of my inspection today is a status check on a preliminary restoration. During my inspection the well is equipped with 8.625 inch surface casing, 4.5 inch production casing, 1.5 inch tubbing, rods, and a pumping unit. The 1.5 inch tubing valve is opened and the 4.5 inch production casing valve is opened. The production equipment consists of three 210 bbl. tanks, 2 vertical separators, and 2 chart meters with chart dates reading 6/7/16. All production equipment will be shared between 29752, 29739, 29740, and 29738. Four frac tanks are on location. The preliminary restoration cant be passed until the frac tanks are removed.
