Currently 94 out of 95 proposed stages to be completed have been stimulated. Nine Energy wireline service is on location. Annular pressure monitoring is in place between the 9.625 and 5.5 casing strings. All stimulation support equipment is situated on polymer liner.
0700415222 2017-11-27 Casing String Witnessed No Violations The purpose of my inspection was to witness the cementing of the 20 inch diameter conductor and the 16 inch diameter mouse hole casing. A 24 inch diameter conductor cemented to surface was in place. During the process of drilling the 22 inch diameter mouse hole the auger bit punctured the side of the 24 inch diameter conductor at 64 feet in depth leaving a hole in the wall of the conductor. Precision Drilling ran a 20 inch diameter conductor to a depth of 104 feet within the 24 inch diameter conductor. A 16 inch diameter mouse hole casing was ran to 68 feet in depth. Pre-job safety meeting was held with all personnel on location including Precision Rathole, Stingray cement crew, and Eclipse. Stingray Cementing hooked up to the 20 inch conductor and pumped fresh water to circulate the hole and the fresh water reached the surface from the mouse hole and the conductor simultaneously indicating that there was a direct line of communication between the two boreholes. Stingray pumped 10 barrels of water, 243 sacks of Class A cement at 15.6ppg, and displaced with 34bbl fresh water. 18 barrels of cement returns were witnessed in the cellar. The annulus of the conductor and the mouse hole were both cemented to surface by pumping through the 20 inch diameter conductor.

1332232794 2018-05-17 Completion Testing No Violations Status check inspection found the well drilling process is complete and the well has the completion well head installed and has also had the 5.5 production string toe prep procedure and final pressure test conducted. The well is awaiting the start of the stimulation process in the next 10 days, Halliburton completion service will mobilize and begin. All completion equipment will be placed on polymer containment. All surface water created on the well pad due to the recent rain events is being removed by the means a vacuum truck. The perimeter drain valves are closed.
1352261395 2018-11-28 Production Wells No Violations Status check inspection found 4 wells are present on the pad and all 4 wells are now in production. The said well is equipped with a lower A and upper B section with annular pressure monitoring in place on both well head sections. No annular pressure was found on this day on the A or B well head section. The well is producing off the 5.5 diameter production casing through (2) 3.5 diameter flow lines with both flow line valves open. The 5.5 casing pressure is 5,263 pounds per square inch (psi) and the flow line pressures are 1,400 psi and 1,500 psi. There is one manual master 7.0625 completion valve still on the well head B section that the well is being produced through. Each of the (2) flow lines has a separate glycol heater and downstream from the Glycol heater is a contact tower and B-Tex Condenser followed by third party mid stream Eureka Hunter pipeline station point. Also on location is a production fluid facility that consists of (6) 400 barrel production tanks situated inside a polymer lined steel containment dike area. Proper well identification is posted and hazard warning labeling is present at the production facility. Signage is posted at the well pad entry point and the well pad perimeter is protected by an earthen berm.
1422104840 2018-01-02 Production Wells No Violations On site inspection to witness the surface casing cement job with H&P Drilling rig 608 on location and Schlumberger cement service. The rig has drilled a 17.5 diameter surface borehole to a measured depth of 1,797 off of a drilling rig Kelly Bushing heigth of 27 above ground level. 1566717453 2018-07-30 Completion Testing No Violations On this day, I accompanied ODNR Inspector William Stolarik. Please reference his comments for a detailed report. We are onsite witnessing Halliburton, 9 Energy, and S3 Pump Service conducting hydraulic fracturing operations. This particular well has 1 stage remaining, of 95 total stages.
1690234680 2018-01-03 Casing String Witnessed No Violations The purpose of my inspection was to witness 1751 feet of 13.375 inch diameter surface casing being cemented into a 17.5 inch diameter. Schlumberger pressure tested their lines and pumped 280 barrels of fresh water, 20 barrels of 6% gel, 10 barrels of fresh water, and 286 barrels of class A cement weighing 15.6 pounds per gallon. A cement sampled was pulled and weighed in pressurized scales confirming the weight of 15.6 pounds per gallon. The top plug was dropped and 262 barrels of fresh water were pumped for displacement. At 200 barrels within displacement cement retuned to surface.
1736874224 2018-08-16 Completion Testing No Violations Status check inspection on the well completion process found the well drilling and completion process now complete and a tubing snubbing unit owned by Deep Well Services rigged up on the well drilling out the 98 completion plugs set in the well during stimulation. The well was stimulated in 99 stages and the snubbing unit is drilling out stage 28. The well head assembly consists of a lower A and upper B section with a production flow cross present. Atop the flow cross is 2 FMC manually operated 7.0625 diameter master valves. Atop the master valves is a completion blow out preventer (BOP) owned by KLX. Atop the BOP is the snubbing unit. All completion equipment working pressure is 15,000 pound per square inch rated. No production tubing will be hung in the well bore at this time. All completion and support equipment is situated on polymer containment. No wells are being produced while the completion plugs are being drilled out. All well pad perimeter drain valves are closed and any surface water created on the pad is being hauled off as precautionary measures. 4 wells are present on the pad and crews are on site completing the installation of the production equipment.
