0067517175 2019-04-06 Production Wells No Violations I arrived to the well location to oversee clean up work from the oil release. Mr. Joe Bauer arrived at approximately 10:00 AM. Mr. Bauer brought a hoe to excavate material and fix the road going up to the well. Mr. Bauer attempted to pump oil from the dike for the 100 barrel shipping tank into a plastic tank but did not have an adequate pump for the oil. I told Mr. Bauer he would need a bigger pump if he wants to do the work himself. Mr. Bauer said he would get another pump and come back on 4/7/2019.

Upon arriving at the well location, I found the 100 barrel load out tank had overflowed oil into the dike. I proceeded to the well location that consists of a pumping unit and 2-100 barrel tanks. There is evidence of brine kill starting at the dike and proceeding downhill into a valley. Samples were obtained using chloride titrators. The samples taken range from 2200 to 3000 PPM chlorides. I spoke with emergency response coordinator Terry Whitehair who is with the Division of Oil and Gas. I reported the spill at 1431 hours.

The area effected is approximately 20 feet by 120 feet at the base of the hill. The area next to the tank battery at the top of the hill is approximately 40 feet by 70 feet. The well consists of a wellhead with tubing and rods and 2-100 barrel tanks. One of the 100 barrel tanks has leaked oil into the containment dike. New pictures were taken during my inspection.

1445615072 2015-10-23 Administrative Inspection No Violations At 11:40 am I spoke with Mr. Joe Bauer (owner) of Highridge Petroleum about the brine kill at the well site, tank battery and area below well site. Mr. Bauer stated he has not pumped the well for several months. There was a leak in the brine tank and he thought he had cleaned up the contaminated soils. I explained to Mr. Bauer he would need to remove all contaminated materials at well site, tank battery and below tank battery and dispose of at an EPA approved landfill. After all contaminated materials have been removed fresh dirt would need hauled in, than seeded and mulched. Mr. Bauer is in Mississippi and will return this weekend; he will contact Inspector Kip Thompson on Monday morning to start clean up.

1528824214 2018-06-12 Production Wells No Violations I met Mr. Jake Glascock and Mr. Nathan Lane on site at 7:45 AM. Mr. Glascock is with ODNR division of oil and gas management. Mr. Glascock job was to sample the brine contaminated soil. Mr. Lane is also with ODNR division of oil and gas management. Mr. Lanes job was to use a soil probe to determine the extent of the contamination. The areas of contamination were measured and the amount of material to be removed was calculated. These calculation were sent regional supervisor Mr. Dan Goins.
1554555524 2019-04-05 Production Wells No Violations I conducted a compliance notice follow-up on this well today. The well was not capable of production as it was not equipped with a pumping unit. I observed that a valve connected to the pumping tee was in the open position and a valve connected to the tubing wellhead was in the closed position. The well owner/operator shall submit a written plugging schedule to Brandon Mitchell at 5880 Memory Road Zanesville, Ohio 43701-9553 within 30 days, per ORC 1509:13(B)(7), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied. Inspector Kip Thompson issued a compliance notice on 10/20/2015 to Highridge Petroleum regarding pollution and contamination at the tank battery. The pollution and contamination has not been remediated or cleaned up. During my inspection of the tank battery near the well I observed that oil has leaked from one of the 100 barrel tanks since my previous inspection on 1/25/2019. The oil has escaped the dike and traveled down hill to a wet marshy area. I also observed that oil has spilled over the top from the 100 barrel shipping tank located at these coordinates: N 39.50744 W -82.31323. A dike for the tank has contained the oil. This tank has a 3 inch plastic load line that crosses a stream and had valve with a bull plug on the end of the line. The valve on the end of the load line was closed. The 3 inch valve on the tank for the load line was in the open position. This could allow the fluid inside the tank to empty directly into the stream if the plastic line would happen to be damaged from stream debris. I contacted Southwest Region Supervisor Eric Conrad at 9:44 AM and advised him of my findings. I then contacted the Emergency Response duty officer line and left a voicemail message with Scott King. At 9:57 AM I left a voicemail message for well owner Joe Bauer notifying him of the oil release. Supervisor Conrad and Emergency Responder Nathan Land arrived at approximately 11:00 AM. Mr.
1555001415 2019-04-09 Production Wells No Violations During my inspection I found Joe Bauer working on the clean up from the oil release. When I arrived a Corp Oilfield Service water truck was taking fluid from the shipping tank and hauling it to a disposal well. Joe Bauer was pumping oil out of the tank battery dike near the well. Joe decided to use the existing plastic lead line that is connected to the shipping tank to pump the oil through out of the dike near the well. Carol Siegley with the Ohio EPA arrived to assess the oil release. Mrs. Siegley determined that the area below the well location that the oil had reached was a wetland area. Mrs. Siegley said she would advise Mr. Bauer of how to proceed with the clean up regarding the wetland area. Emergency Responder Nathan Lane was also present during the inspection. Mr. Bauer was working on removing oil from the dike at the tank battery near the well and the dike for the shipping tank. Mr. Bauer has also removed a good amount of oil from the wetland area by using absorbent pads. Mr. Bauer is making good progress on the clean up. Time of inspection: 9:15 AM.

Mr. Bauer arrived with a 2 inch trash pump and pumped the oil from the shipping tank dike. Mr. Bauer then placed absorbent pads on the oil at the bottom of the hill below the well. Mr. Bauer is waiting for instructions from Ohio EPA on how to best clean up the oil from the area below the well as it may be a wetland area. Mr. Bauer said he would be back in the morning with more absorbent pads. A water vacuum truck is scheduled to be here as well in the morning to take fluid from the 100 barrel shipping tank and the oil that was pumped out of the dike. Time of inspection: 9:10 AM.
