At the time of this inspection, I observed the following: The wellhead plunger lift and associated tank battery located in the back of a residential property. At the time of this inspection, I observed the following: The enclosure gates were locked. The load lines were plugged and secure. The secondary containment was intact and dry with no signs of leaks or spills. Fittings and connections at the wellhead and battery were secure and free from leakage or problems. I walked the areas surrounding the wellhead and battery, and found no signs of impact or degradation attributable to the production system. The surrounding, shared-use, gravel-paved access road was in good condition.
0304968029 2015-02-27 UrbanProduction Wells No Violations I inspected this well as part of routine inspection activity. The wellhead and battery are located on a single-family residential property on the north side of Wilson Mills Road. The wellhead plunger lift and battery are in separate board-on-board enclosures which are locked. I observed a minor leakage of gas from a fitting. Fittings and connections at the battery are secure and free from signs of leakage or problems. The load-outs are secure, plugged, and locked. Process safety gear is present. Signage is in place and current. At the time of this inspection there are 15 to 20 inches of snow cover in the area which obscures some surficial elements from observation. I contacted the well tender (Gary) and advised him about the minor gas leak. He indicated that he would be back up to the unit on 3/1.
0546208972 2018-07-05 UrbanProduction Wells No Violations I inspected this well location as part of routine inspection activity. At the time of my inspection, I found the following: fittings at the wellhead were secure. The installation of the pump jack system had been completed and the well was in production. I walked the areas surrounding the wellhead and battery and found no indications of problems.
0988628988 2017-11-17 UrbanProduction Wells No Violations I inspected this well pursuant to Complaint 5760. Registered by Highland Heights Fire Chief Turner. Chief Turner had fielded several citizen calls regarding gas odor in the vicinity of this well. FD crews found that Cutter Oil was performing work-over activities on the well, including removing the plunger-lift unit and replacing with a pumping unit. I met with CJ Cutter and crews from Hagen Well Service at the location. CJ reports that there had been some gas vented as they performed these activities. I reported my findings back to Chief Turner. He requested that, as a courtesy, well owners make prior notice, when possible, of these types of activities. I relayed this request to CJ Cutter.

1355699927 2017-12-07 Work Over Reconditioning No Violations I inspected this well location as part of routine inspection activity. At the time of this inspection, crews were working to begin running pump rods, ahead of setting a pumping unit. Crews had staged rods and tubing on flat-beds. The well was shut-in with no observed problems. The well enclosure was dismantled during work-over. Fittings and connections at the battery were secure with no problems. I had previously notified the Highland Heights Fire Chief that the work would be taking place and there might be some gas odor reports associated with the work.

At the time of this inspection, I noted the leak had been repaired. The well was in active production. The enclosures were secure with no signs of problems.
