0818814699 2011-11-16 Production Wells No Violations Called out to inspect well per request of ODNR hydrogeologist Kyle Tharp due to nearby water well showing high levels of natural gas. Well head and tank battery properly identified. No visible or audible signs of leaks or spills. No gas odor detected. Met Allen Summers of Enervest Operating on location to witness pressure reading on the 8.625 casing annulus. Partial excavation around well head revealed that no 8-5/8 casing is exposed. Instructed Enervest to fully excavate cellar around well head and clean all soil from casing to fully expose 4.5 and 11.75 casing. Will make determination of further actions upon completion of excavation.
1167367137 2009-07-01 Production Wells No Violations I walked around the tank battery and the wellhead and found that there were no violations at this time.
1168164158 2011-11-21 Production Wells No Violations Complete excavation of the well head revealed that the 8.625 and 4.5 casing were both on a double-bowl casing hanger. The 8.625 casing has zero (0) pounds per square inch (PSI) annular pressure, as this casing is open to atmosphere. No violations at this time.
