Inspections Performed

Inspection ID Inspection Date Inspection Type Inspection Result Inspection Comments
1391014072 2014-01-28 Surface Facility Construction No Violations On the day of my inspection I met with the landowners involved in this well. Eclipse was preparing to build a lease road and pad location to drill a well. The landowners were wanting to know if they were within compliance and able to drill the well. I explained to them that Eclipse was within compliance from the States stance according to ORC 1509 they were permitted to carry on with their operation. We also discussed erosion control. No violations noted.
1430069159 2014-01-29 Administrative Inspection No Violations I contacted Tom White via cell phone and informed him of my meeting with the landowners. I advised him to place proper erosion and sediment controls throughout the area. He agreed and told me that he would make sure they completed the construction properly.
