0124137296 2013-03-07 Completion Testing No Violations I was on location to check the status of this shut-in, horizontal well. The surface equipment was in place and the production unit, tanks, and wellhead had been plumbed together. Electric power was being run to the location. The cellar was full of water and the wellhead was chained down. No violations.
0171734541 2012-08-15 Drill / Deepen / Reopen No Violations I was on location with inspectors Tom Hill, Bob Roush, John Fleming, Steve Tompkins, and Ryan Channell to witness cementing of the conductor pipe and review drilling plans for Mountaineer Keystone (MK). Others in attendance were Ross Schweitzer, Jim Watson, and Bill Ingalls from (MK), Union Drilling, and other contractors. The plans were reviewed and discussed so that everybody involved was on the same page and understood what was required by the Division of Oil and Gas. For the conductor cement job, Butchs Rat Hole had augered a 30 hole to 63 and set 63 of 20 conductor casing in the hole. Baker Hughes rigged up, established circulation, and cemented the casing with 195 sacks of Type 1 cement. Good cement returns were achieved and held at the surface. 0323510688 2013-08-08 Preliminary Restoration No Violations I was on location to inspect the site for preliminary restoration. The sound walls have been removed and the ditches around the pad have been taken out. The west side of the pad has been reclaimed, graded, and seeded. Vegitation is well established around the perimeter of the location. Preliminary restoration is approved.
0498743429 2013-03-28 Completion Testing No Violations I was on location with North Region Supervisor Tom Hill to inspect the flow back equipment and set up at this horizontal well. Tim Altier (Mountaineer) and Garrettsville and Windham Fire Departments were on location as well. Rockwater Energy was assembling the flow back equipment and Tim led us around the location discussing the testing process and safety measures installed on the equipment. All gas and vapors will be incinerated on location as they are produced during the flow back. Testing will begin Tuesday morning. No violations.
0621413011 2013-01-31 Production Wells No Violations I was on location to check the status of this horizontal well. ROC Well Services was on location constructing the surface facilities and equipment. The tank battery was constructed and contained 12-400 barrel tanks. The spacing between the tanks meets the requirements of at least three feet. The tank battery was diked with a 4 steel wall. The entire dike was lined with a heavy duty liner and was riveted to the top of the steel wall. Pea gravel was placed on top of the liner and the tanks were set on steel rings filled with gravel. The productions units had no been spotted. The wellhead was chained and locked and the cellar was full of water. No violations.
0735682431 2014-02-07 Production Wells No Violations I was at the compressor station along the haul road to check the status of this well and the Soinski #202 (#24441) being placed into production. The pipeline has been hydro-tested and they are currently testing the water to determine if it can be discharged. The wells are not yet in production. No violations.
0742423072 2013-01-16 Administrative Inspection No Violations I was on location with Supervisor Tom Hill, Inspectors Ryan Chanell and Kevin Perry, and Tim Altier (Mountaineer Keystone) to inspect the annuli on this well. Mountaineer removed the pea gravel from the cellar to expose the cut-off casing. The annulus between the intermediate casing and the surface casing had minimal bubbling when water was placed over the cement. The other strings showed no signs of gas-cut cement. No violations.

This plug was set through 2.75 tubing in two stages and covered the zones from 6186-5174 measured from the kelly bushing. Baker-Hughes rigged up and pumped 386 sacks of cement to complete the job. 1226227774 2013-06-20 Production Wells No Violations I was on location to check the status of this horizontal well. The well was shut-in at the time of my inspection and all of the flow-back and testing equipment had been removed. Two steel tanks were on containment and being cleaned prior to removal. The 6 pipeline has been installed and buried south of the location and tied into a compressor station that is being constructed. The station is located east of the main building as you enter down the haul on the property. No violations.

1349281439 2012-08-20 Drill / Deepen / Reopen No Violations During this status check inspection, I found that the security shack had been placed at the entrance and the sounds walls were constructed. Containment had been laid below the drilling area and the rig sub had been spotted on the containment area. The derrick was on location and Union was waiting on equipment to begin assembling the rig. I spoke with Ross Schweitzer (Mountaineer Keystone) about the assembly time-frame and he thought it would be a week until they were ready to spud the surface hole. No violations.
1349295215 2012-09-08 Drill / Deepen / Reopen No Violations I was on location at 4:15am to witness the cementing of the intermediate string. Union Drilling drilled a 12.25 hole to a total depth of 4280 measured from the kelly bushing (KB +16). Union drilling ran 4246 (KB) of 9.625 casing. Baker Hughes rigged up, circulated the hole, and cemented the casing with 1319 sacks of Type 1 cement. 50 barrels of good cement returned to surface.
1350478068 2012-09-12 Drill / Deepen / Reopen No Violations I was on location to witness the testing of the blow-out-preventor (BOP). Greenes Energy Group was on location to conduct the test. Greenes tested the first set of pipe rams and inside kill valve. The rams and valve were tested at 250 and 4000 psi and held for 5 minutes with no leak off. The remainder of the tests were delayed until all the bolts on the stack could be tightened and I did not witness the remaining tests. I told Bill Ingalls (Mountaineer Keystone) I would be back to pick up a copy of the BOP test charts. During the delay, I walked the outside perimeter of the well pad to inspect where rain water was being discharged out of a ditch on the west side of the pad. I could see where a 4 line had been placed in the grass and there was a compost filter sock placed just into the bean field to prevent erosion. No violations.
1355842439 2012-12-03 Completion Testing No Violations I was on location to check the status of completion operations and hydraulic fracturing of this horizontal well that began this morning. I met with Jeff Wozniak and Jake Smathers (Mountaineer Keystone) to observe the completion operations. A 32 sound wall had been constructed inside the 20 sound barrier on the east and north sides of the location in an attempt to mitigate noise emanating from the equipment on location. U.S. Well Services was rigged up and pumping water down hole to fracture the Point Pleasant Formation. Jeff told me they were unable to put away any sand during this stage and that it was a slick-water frac. Containment was in place around the wellhead and the location was in good condition. The annulus was being continuously monitored and there were no signs of pressure during completion. No violations.
1365775519 2012-08-29 Drill / Deepen / Reopen No Violations I witnessed Baker Hughes cement the surface casing for Mountaineer Keystone. Union Drilling drilled a 17.5 Hole #2 to 422 and set the 13.375 surface casing at 392. Mountaineer Keystone used 54 lb. casing. Baker Hughes broke circulation and pumped 386 sacks of Class A cement. They displaced with 62 barrels of fresh water. There were approximately 29 barrels of good cement return to surface.
1377106732 2013-07-15 Preliminary Restoration No Violations I was on location to check the status of site reclamation. Some of the sound wall has been removed from around the location. Canton Oil Well Services was on location painting equipment. Pressure on the wellhead was at 2200psi. Mountaineer plans on removing the walls and ditch around the location in the next several weeks and reclaim the west side of the pad. Restoration is progressing. No violations.

1433874219 2015-04-29 Production Wells No Violations I was on location to check the status of this Utica Shale well. This well remains shut-in due to pipeline tie-in issues and has only produced during flow back operations conducted in the second quarter of 2013. Gauges indicate 2090psi on the tubing and 1190psi on the casing. The wellhead is contained within a liquid tight cellar and the 13.375 surface casing annulus is open to the atmosphere. The well is plumbed to a fire heated, gas production unit and is connected to two (2) 400 barrel tanks inside a containment barrier. The cellar is covered with steel grating and there are concrete barriers around the wellhead. The gate valves on the wellhead are chained and locked and there is a glycol injector on the west side of the wellhead. IDs are posted at the wellhead and the lease road entrance. The lease road is gated and locked. There is a pile of compost filter socks and orange safety fence on the north side of the pad and a pile of plastic liner on the south side of the pad. I will ask Mountaineer Keystone to remove the unused equipment, about their plans for the well, and to consider applying for temporary inactive status (1st year) to comply with Chapter 1509.062 of the Ohio Revised Code. No violations.
1524435521 2012-08-23 Administrative Inspection No Violations I arrived on location for a pre-spud meeting with Mountaineer Keystone and all of their contractors. I met with on-site well consultants Bill Ingalls and Frank Covine to discuss when I should be notified for cementing and other operations and their plans for drilling the surface hole. We exchanged contact information and I left location. Union Drilling was still rigging up. No violations.
1528709221 2012-09-24 Drill / Deepen / Reopen No Violations I was on location to pick up the plug-back ticket and inspect the drilling operations. Union Drilling had tagged the kick plug at a depth of 5177 measured from the kelly bushing (KB). They drilled the plug to the kickoff point of 5379 (KB) and were beginning to build angle towards the horizontal leg of the well. All drilling equipment is on containment and the location looks good. No violations.
1660602637 2013-04-09 Administrative Inspection No Violations I was at Mountaineer Keystones office with Deputy Chief Bob Worstall and Engineer Blake Arthur to review plans for a proposed freshwater impoundment to the south of this well pad. We met with Tim Altier, Brad Litman, Jack Conway, and Jaime Johnson. We received a copy of the engineering schematics and reviewed them with Mountaineer. We went to the proposed location of the impoundment in the afternoon and were joined by North Region Supervisor Tom Hill. The site was staked and located in an agricultural field. The plans were reviewed again and Blake would review the plans in detail in Columbus.
1693049756 2013-05-16 Completion Testing No Violations I was on location to check the completion testing status of this well. Completion testing was finished and all temporary flaring and flowback equipment has been removed. There are seven empty frac tanks sitting on containment. The site is clean and the wells are shut-in. 6 steel pipeline has been laid out from the southeast corner of the pad and runs south across the property. No violations.
1709605752 2012-12-13 Completion Testing No Violations I was on location with North Region Supervisor Tom Hill to check the completion status of this horizontal well. We met with Jeff Wozniak (Mountaineer Keystone) in U.S. Well Services (USWS) communication trailer. USWS was rigged up and preparing to pressure test the lines prior to hydraullically fracturing the last stage of the Point Pleasant Formation. Jeff told us that the annulus has never shown any pressure and is being monitored during completion operations. He said after they complete this stage, they will finish the final stage of the #202 and rig down. They will then rig up the coiled tubing unit and drill out the kill plugs. After that they will bring in a snubbing unit to run tubing in both well bores and shut the wells in for 90 days prior to testing. Containment was down and the location was in good condition. No violations.

1790824379 2013-01-15 Administrative Inspection No Violations I was on location with Supervisor Tom Hill, Inspector Kevin Perry, and Mountaineer Keystone (MK) Operations Manager Tim Altier to check for any annular pressure on this well. All casing strings in this well were cemented to surface and the valve between the 5.5 and 9.625 was open and showed no signs of pressure. The cemented annuli of the cut-off casing strings were buried under pea gravel and there was a yellowish-brown fluid at the bottom of the cellar that appeared to be slightly bubbling. We requested that the buried casing strings be exposed and Tim told us he would have them exposed tomorrow. No violations.
1796139749 2012-09-14 Drill / Deepen / Reopen No Violations I was on location to pick up the results of the blow-out-preventor (BOP) tests. A review of the test results showed that all tests passed. Union Drilling was drilling ahead at a depth of 5619 at the kelly bushing and preparing to core the hole. No violations.
1838214474 2012-12-06 Completion Testing No Violations I was asked by the Deputy Chief, Bob Worstall, to investigate claims of a resident of Frazier Road that there was a burned area outside the northeast corner of the well pad caused by pumping water out of a ditch that surrounds the location. I initially investigated this complaint about pumping water out into the bean field on 9/12/2012. At that time it was determined that it was only rain water from the ditch. I investigated the area in question and found no signs of burned grass. There was a patch of dried up beans at the northeast and northwest corners of the well pad. The beans were not harvested due the presence of compost filter socks to control off-site erosion. Grass and beans on and around the stockpiled topsoil, near the entrance to the well pad, had the same appearance as beans found in the area in question. No burned area located. No violations.
1891782666 2012-10-09 Drill / Deepen / Reopen No Violations I was on location to pick up the cement ticket for the production casing cement job completed by Universal Well Services on 10/07/2012. Union Drilling was in the process of skidding the rig to the Soinski #202 (API# 34133244410000) conductor hole on this location. The rig mast was down and they were working on the jets in the steel pits. A part on the travelling block was broken and would not be fixed until tomorrow. No violations.
