| Mr. Brewer and I viewed and discussed the drill cuttings processing.  The current treatment system in place consists of (2) rig shakers, (2) drying shakers, (1) centrifuge and a pug mill unit.  The centrifuge provided by Peak Energy Services, a Clean Harbors company, extracts the solids from the liquids in the drill cuttings, removing only low gravity solids.  The liquid material is reused as a drilling fluid on-site upon returning to the active drilling system or it is stored on location in frac tanks and sent to another Chesapeake location as the rig moves to be reused for drilling purposes. There are no chemicals added into in the process.  An alternative waste management practice (OG-71) application needs to be submitted to the Department in relation to the process described above.

| During this inspection, Ms Bowen, Apex inspector contracted by Chesapeake, arrived on location. I asked her about the produced water tank since it did not have secondary containment. She said that it was double-walled which eliminates the need for secondary containment. The produced water tank is in good condition overall.

| Mr. Brewer and I viewed and discussed the drill cuttings processing.  The current treatment system in place consists of (2) rig shakers, (2) drying shakers, (1) centrifuge and a pug mill unit.  The centrifuge provided by Peak Energy Services, a Clean Harbors company, extracts the solids from the liquids in the drill cuttings, removing only low gravity solids.  The liquid material is reused as a drilling fluid on-site upon returning to the active drilling system or it is stored on location in frac tanks and sent to another Chesapeake location as the rig moves to be reused for drilling purposes. There are no chemicals added into in the process.  An alternative waste management practice (OG-71) application needs to be submitted to the Department in relation to the process described above.

| During this inspection, Ms Bowen, Apex inspector contracted by Chesapeake, arrived on location. I asked her about the produced water tank since it did not have secondary containment. She said that it was double-walled which eliminates the need for secondary containment. The produced water tank is in good condition overall.

Four wells are staged for production with four GPUs and four sand separators. Berms surround the perimeter of the pad and pad side slopes are well-vegetated with greater than 70% vegetation. No PCSM structures observed. No E&S issues noted.

2224962 Primary Facility 2013-11-22 Drilling/Alteration No Violations Noted Mr. Brewer and I viewed and discussed the drill cuttings processing. The current treatment system in place consists of (2) rig shakers, (2) drying shakers, (1) centrifuge and a pug mill unit. The centrifuge provided by Peak Energy Services, a Clean Harbors company, extracts the solids from the liquids in the drill cuttings, removing only low gravity solids. The liquid material is reused as a drilling fluid on-site upon returning to the active drilling system or it is stored on location in frac tanks and sent to another Chesapeake location as the rig moves to be reused for drilling purposes. There are no chemicals added into in the process. An alternative waste management practice (OG-71) application needs to be submitted to the Department in relation to the process described above.

2794280 Primary Facility 2018-10-23 Routine/Complete Inspection No Violations Noted During this inspection, Ms Bowen, Apex inspector contracted by Chesapeake, arrived on location. I asked her about the produced water tank since it did not have secondary containment. She said that it was double-walled which eliminates the need for secondary containment. The produced water tank is in good condition overall.

Four wells are staged for production with four GPUs and four sand separators. Berms surround the perimeter of the pad and pad side slopes are well-vegetated with greater than 70% vegetation. No PCSM structures observed. No E&S issues noted.

Two compressor units are present on the pad.
