Seven wells are drilled (1H, 2H, 3H, 4H, 5H, 6H, and 8H) and one additional well is permitted (7H). The Fox pad has not yet been constructed. The erosion control BMPs were inspected and wer found to be functioning and maintained. Several puddles around the pad were monitored with a conductivity probe. No elevated readings for specific conductivity were found outside containment. A reading of 3,200 µS was taken from inside the secondary containment for the production tanks. This material should be recovered and disposed of properly. Both cut and fill slopes were showing signs of sporadic vegetative growth. These areas need to be assessed to determine if re-seeding is warranted. No violations were cited during this inspection. - MRN

2210695 Primary Facility 2013-10-07 Routine/Partial Inspection No Violations Noted On October 7, 2013 an inspection was conducted at the Pierson 810 pad in Gaines Township, Tioga County. The last inspection conducted on behalf of the operator was on 10/4/13. During this inspection the following was noted: The silt sock installed at this has degraded over time and is in need of maintenance or replacement prior to any additional earth disturbance activities. Several area of the site are in need of additional stabilization. No evidence of sediment transport off-site was observed during this inspection. Several puddles were screened and no exhibited elevated conductance. No violations were cited during this inspection.
2468106 Primary Facility 2016-03-17 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) inspected the SWEPI LP – Pierson 810 site on 3/17/2016 @ 1300 hours. The weather was 54°, partly sunny 41% humidity. An adequate site ID sign was observed at the entrance of the access road. A copy of the well permits was posted on the back of the sign. The site was last inspected by company personnel on 3/9/2016 with “no visible issues” noted in the comments section. The Pierson 810 pad is located ~6500’ past the Thomas 808 pad, along the same access road. The access road appears stable. A compressor station is located on the pad. Eight well heads were visible on the pad surface during the inspection; four appear to be in production. The cut slope of the pad appears well stabilized and well vegetated. The cut slope appears rocky with vegetation in some areas. Silt sock was observed below the fill slope. No fluid was observed in the secondary containment surrounding the produced fluid tanks. No violations are noted at this time. I departed the site @ 1345 h
2624226 Primary Facility 2017-08-09 Site Restoration No Violations Noted On 8/9/17 at 1240 hrs I conducted an inspection of the Pierson 810 well site. Dan Oakes, SWEPI Production Operator, arrived during the inspection. It was mostly sunny and 78 degrees F.

I observed the site and reviewed the erosion and sedimentation (E&S) routine inspection log, the Notice of Termination (NOT) and the Well Site Restoration Report (WSRR).

The site consists of the access road and well pad. Eight wells, the Pierson 810 1H - 8H, (Photos #1 & 2), have been drilled. According to DEP records, the 1H, 3H, 5H and 6H wells are in production. The 2H, 4H, 7H and 8H wells were placed on inactive status on 3/14/12.

Produced water is stored production tanks, which are staged in secondary containment (Photo #3). Routine secondary containment inspections are being completed; the last inspection was on 8/9/17. Water in the containment structure had a field measured specific conductance reading of 763 micro Siemens/cm.
