0139267409 2018-01-04 Drill / Deepen / Reopen No Violations The well is being hooked up to the Columbia gas pipeline. Kokosing/Integrity is the contractor performing the work.
0225701889 2017-11-20 Drill / Deepen / Reopen No Violations Production casing was cemented on Friday, 11/17/17. The operator did not run a latchdown plug behind cement and no cement was left above the shoe. When they attempted to remove the cement head, the well began flowing back. The operator believes this to be caused by reservoir pressure pushing the plug up hole. They have not run a bond log yet because they cant remove the cement head from the 7 inch casing. No gas pressure is present on the backside of the 7 inch. The well was shut in with the cement head after the wiper plug was pumped back down. They are planning to freeze the water in the 7 inch above grade, remove the cement head and install a valve to maintain well control. Wild Well Control is scheduled to conduct the freeze.
0327929508 2018-05-29 Preliminary Restoration No Violations A contractor for Columbia has begun resoiling and finish grading the location. The location is about half finished. No personnel were working during inspection.

0452128877 2017-12-05 Drill / Deepen / Reopen No Violations The company rig has drilled out the wireline bridge plug and started drilling out the packer. The mud pump developed a leak and had to be shut down. The rig is waiting on their other mud pump to be delivered to the location to resume drilling.
0545676245 2017-12-04 Drill / Deepen / Reopen No Violations Columbia Gas started drilling out the well. They are using a downhole motor for the drill out. The motor requires 100 to 200 barrels per minute of fluid rate to operate. The meter on the mud pump was not operating properly to indicate fluid rate and needed to be repaired.
0676065298 2017-10-19 Drill / Deepen / Reopen No Violations HAD Drilling is on location with a dual rotary rig setting conductor casing. Terry VanSickle of Columbia Gas is on location. 20 inch 78.6 pound collarless casing is being installed. The rig does not leave an annulus around the casing so it is not cemented. The casing is being drilled to 160 feet.
0728502917 2017-11-29 Drill / Deepen / Reopen No Violations The Columbia Gas service rig is on the well. They are going to fish the bridge plug and drill out the well. They will bond log the 7 inch casing following drill out.
0920913537 2017-12-20 Drill / Deepen / Reopen No Violations The operator is attempting to conduct flow testing on the well. The choke manifold ahead of the flare stack froze or plugged up with debris from the well bore. Testing will be completed when equipment is repaired.
0956281063 2017-11-02 Drill / Deepen / Reopen No Violations The rig is drilling on saltwater polymer mud at 1600 feet.
0998686515 2017-11-22 Drill / Deepen / Reopen No Violations HAD Drilling is moving the rig off location. The well will be completed with a service rig.
1135736163 2017-10-28 Casing String Witnessed No Violations The operator requested inspection to witness cementing the surface casing in the well. The job was scheduled for 10 AM and backed up until 1 PM. On arrival, the rig was running surface casing. The rig had difficulty installing casing in the well and the job started at 4:45 PM.
1171902157 2017-11-16 Drill / Deepen / Reopen No Violations HAD has reached total depth and they are tripping the bit out of the hole. Total depth is 4928 feet with 2200 feet of lateral.
1177706198 2017-10-31 Drill / Deepen / Reopen No Violations HAD Drilling is repairing a mud pump at the rig. Drilling has not resumed below surface casing yet. Rick Conley of Columbia Gas on location.
1343127451 2017-10-24 Drill / Deepen / Reopen No Violations The conductor rig has been moved off location. HAD Drilling is moving in support equipment for the primary rig. Rick Conley of Columbia Gas is on location.
1375211074 2018-03-23 Preliminary Restoration No Violations The operator is restoring the drill location. The site is rough graded and the road has been surfaced with limestone. All of the rig mats and liner have been removed. Topsoil needs to be spread on the location. The well has been connected to the pipeline and is equipped with a separator. ID needs to be posted at the well. No personnel were on site during inspection.
1506459541 2018-06-12 Preliminary Restoration No Violations Workhorse Excavating is on location. The area has been topsoiled and finish graded. The contractor is working on the access around the wellhead. Silt fence remains in place around the location.
1511289019 2017-11-21 Drill / Deepen / Reopen No Violations Wild Well Control, Key Energy Service and Baker Hughes are on location. HAD Drilling rig is on the well. The fluid inside the 7 inch casing was frozen by liquid nitrogen to control the well to permit removal of the cement head. After the ice was pressure tested, the cement head was opened. No flow was observed and the head was removed. A gate valve and flange was installed on the 7 inch casing. The operator rigged up to melt the ice plug from the 7 inch casing with a heat plant. Baker Hughes was then going to install a wireline plug in the 7 inch casing to permit removal of the BOP and landing joint. The autofill shoe was not closed prior to cementing and the wiper plugs pumped to activate the formation packer and behind cement were not latchdown plugs. As a result reservoir pressure was able to lift the plugs off their seat allowing gas pressure to push fluid from the well. The cement head was left shut in for 24 hours following the cement job on 11/17/17.
1512052334 2017-11-09 Drill / Deepen / Reopen No Violations HAD Drilling drilled cement to the base of the intermediate casing. The kick plug top was at 2356 feet. They are drilling to 2372 and then will rebuild curve into the Clinton formation.

The well is equipped with a flanged wellhead with 1-master valve and 1-wing valve, and a drip. Well identification is clearly displayed. No problems were noted at the time of the inspection.
1588332105 2017-11-08 Drill / Deepen / Reopen No Violations The operator requested inspection to witness setting a kick plug in the well. The bit did not achieve enough curve to stay within the Clinton formation and the hole was plugged back to the intermediate casing and will be redrilled. 365 sacks of Class H cement were pumped for the plug. C&J Energy Services did the cementing. The plug was set from 3213 to 2366 feet. Approximately 200 feet of horizontal bore was drilled below the Clinton.
