0956584276 2012-10-16 Drill / Deepen / Reopen No Violations On the day of my inspection I found that sack cement had been mixed and poured down the annulus of the 8.625 casing and 11 hole to fill it to surface from the bridge depth of approximately 20. The pits were still open and there were no sags or tears in the pit liners.
1186098361 2012-09-12 Drill / Deepen / Reopen No Violations On the day of my inspection Petroset started pumping cement down the annulus of the 8.625 casing and 11 hole. The annulus bridged off within 5 minutes of starting the job. Attempts were made to clear the bridge, but were unsuccessful. Appalachian Well Surveys ran a bond log, which showed that there was good cement bond from 672 to 98. Petroset conducted a pressure test of the casing at 300 psi for 30 minutes. The casing passed the pressure test and Maric Drilling was cleared to continue drilling. They will attempt to cement the remaining 98 of open 8.625 casing at a later stage in well construction.

The fluid level was approximately 18 from surface and there were no sags or tears in the pit liners. The well head was shut in with a 4.5 bull plug, and there were no leaks or spills. The drilling rig was no longer on location. Some drilling equipment remained.
1483704917 2012-09-10 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Maric Drilling was rigged up, the pits and flow ditch were lined, and there was new 8.625 casing on the rack. The permit was in the doghouse and there were no leaks or spills. The well was not spudded at the time of my inspection.
1717091810 2012-09-11 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Maric Drilling drilled an 11 surface hole on fluid to 732. They ran 8.625 casing to 672. The hole was circulating with the mud pump when I arrived. Petroset broke circulation, pumped 400 lbs. gel, 100 lbs cotton seed hulls and 260 sack of class A cement. Good circulation was maintained throughout the job and cement was circulated to surface. When Petroset stopped pumping the cement immediately fell down the hole. The height of the cement could not be determined from surface. A collective decision was made to let the cement set overnight and come back in 8 hours to fill from the surface.
1822336479 2013-07-03 Preliminary Restoration No Violations On the day of my inspection I found that the well site was graded out and 90% covered with well established grasses and field vegetation. The leases road was graveled all the way back to the well. The well head was equipped with a pumping unit and connected to a tank battery that consisted of one 210 barrel stock tank, one 100 barrel brine tank, one separator and an earthen dike. Proper identification was posted and there were no leaks or spills. This well site passes preliminary restoration.
2075161272 2012-09-14 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Maric Drilling was drilling on fluid at a depth of 2180. The permit was on display in the doghouse, there were no sags or tears in the pit liners and there were no leaks or spills.
2114814888 2016-05-23 Production Wells No Violations On the day of my inspection I found that the well was being produced with an electric powered pumping unit. The valves were open on the tubing and casing lines. The gauges showed 25 psi. gas pressure. The tank battery consisted of one 210 barrel steel tank, one 100 barrel steel tank, one vertical separator and meter box. An earthen dike was in place around the production equipment. Identification was posted at the tank battery. I found no leaks or spills at the wellhead or production equipment.
