At the time of this inspection, the well consisted of a 4.5 inch casing swaged into a 2 inch line with a regulator. The casing valve and 2 inch line off the 4.5 inch casing was open. The well is primarily used for domestic purposes on this parcel. No effort to mitigate the leaking 100 BBL tank adjacent to the well. As requested in the previous inspection reports, the disconnected 100 BBL tank needs to be removed from the site. No legible identification has been installed at the well or shared tank battery. The tank valve that was dripping brine at the 210 BBL production tank has not been repaired, no has any identification been installed at the tank battery.
1341336485 2019-02-13 Production Wells No Violations I conducted a compliance notice follow--inch ball valve that was closed. Well was disconnected from the flow line. From the top of the valve ran a one-inch steel line over to a two-inch poly line which was directed south towards another well #24213. The tank battery consists of one 100 bbl. production tank and one 210 bbl. production tank, and one vertical separator, value was shut at separator. No Proper identification was posted at the well and tank battery.
1430758834 2015-04-22 Production Wells Violations Noted Complaint# 5010, Jake Ward. My inspection of the well found there is production casing only with a domestic hook-up directly from production casing. There is a 100 bbl steel tank located fifthteen feet south of well. The tank is not connected to anything. There was a puddle of water three feet south-east of well with a small amount of oil floating on it. There was no identification posted at the well location. This well shares a common tank battery with permit# 4213. The tank battery had no identification posted for this well. The last reported production for this well was 32 bbl of oil, 30 mcf of gas and 35 bbl of brine in 2009. There is a unresolved Chiefs Order for failure to maintain liability insurance issued on 7-14-2014. REMEDIAL WORK REQUIRED TO RESOLVE VIOLATIONS: REMOVE SMALL AMOUNT OF OIL NEAR THE WELL,OBTAIN LIABILITY INSURANCE, POST IDENTIFICATION AT THE WELL AND TANK BATTERY LOCATIONS, SUBMIT PRODUCTION REPORTS FROM 2010 TO PRESENT.
1441643106 2015-07-06 Production Wells No Violations Follow-up: Complaint # 5010, Jake Ward. My inspection found there has been no action to correct the compliance issues. I attempted to contact Charles Moisio by telephone to receive no response.

He stated that he is not able to produce the well because the sales lines have been shut in, and are not accessible and that this well was primarily used for domestic gas. The leaking 100 BBL tank on site, adjacent to the well has been disconnected but still needs to be removed as requested in the previous inspection report. No signage has been installed at the wellhead and is needed due to a shared tank battery. I also spoke with Mr. Moisio about removing the leaking 100 BBL tank and adding signage to the well location.

The well is primarily used for domestic purposes on this parcel. The leaking 100 BBL tank on site, adjacent to the well has been disconnected but still needs to be removed as requested in the previous inspection reports. The shared tank battery still had a tank valve that was dripping brine at the 210 BBL production tank. The tank battery was diked and shared with well 2-4213. No identification has been installed at the well or tank battery.
1621077170 2018-02-14 Production Wells No Violations I inspected this well as a follow-up to Chiefs Order #2014-202 for failure to maintain liability insurance. At the time of this inspection, there was a 4.5-inch casing with 4.5 to 2-inch reducer swage, with valve and regulator. The flow-line connection was closed. There was a 100-barrel steel stock tank located adjacent to the well, and a two-tank battery with separator located at the lease road entry point (presumed to be shared with the Brainard (#2-4213) located further south along the same access road. There were no signs of active production. There was no identifying signage. There remained no active liability insurance on file for Plateau Energy Corp.
1727900501 2017-07-12 Production Wells No Violations I conducted a compliance notice follow-up inspection from the last inspection report numbered 1441643106, and complaint 5667. At the time of this inspection, the well consisted of a 4.5 inch casing swaged into a 2 inch line with a regulator. The casing valve was open. There has been no reported production for this well since 2009. I contacted Mr. Charles Moseo of Plateau Energy about the production. He stated that he is not able to produce the well because the sales lines have been shut in, and are not accessible and that this well was primarily used for domestic gas, however the property receiving gas is currently vacant. The leaking 100 BBL tank on site, adjacent to the well has been disconnected but still needs to be removed as requested in the previous inspection report. No signage has been installed at the wellhead and is needed due to a shared tank battery. I also spoke with Mr. Moseo about removing the leaking 100 BBL tank and adding signage to the well location.
