0515742319 2013-07-25 Production Wells No Violations I was on location to follow up on a tank explosion that occurred on 7/10/13. All of the impacted soil has been removed from location and the charred trees have been cut down and stacked on the east side of the location.The site has been graded and the dike and tank battery are being rebuilt. There is new stone inside the dike and in the parking area around the battery. A new 210 barrel tank has been spotted inside the dike and a rebuilt, horizontal separator has been placed inside the dike as well. The wellhead has been rebuilt and the well is shut-in and waiting to be plumbed into the separator. The siphon dam has been removed and all impacted areas on the location have been properly cleaned as well as in the drainage area near the trees.. There is no sign of oil impact on location. The well will need to be tied in to the separator and tank and the dike wall completed. On site clean-up is satifactory.
0642874641 2013-07-10 Production Wells No Violations I received a call from John Murphy (Equity Oil & Gas) at 4:34pm about a lightning strike at this location. He told me the 210 barrel tank was hit by lightning, exploded, and caught fire. I asked him if any product got off location and whether it needed reported to the OEPA. He told me that he was not on the location but by all accounts of people on location, it had all burned up in the fire. I arrived on location at 5:40pm and Northeast Oilfield Services was leaving with a load of 70 barrels of oil, water, and mud recovered from inside the dike of the tank battery. W. Pol Contracting was on location to proceeding with clean-up and containment operations. The site is in a cornfield and the tank exploded and launched roughly 150 yards to the west of the well and into the cornfield. There was no oil in the cornfield to the west or near the tank. The bottom of the tank blew out and was lying just outside the dike when I arrived. The separator and well head had burned and were shut--15 small to medium sized trees were charred in the fire. Estimates place the release between 80-90 barrels, most of which was incinerated in the fire. The contractor constructed a siphon dam just northeast of the tank battery and I instructed them to place booms across the drainage outlet in the woods to prevent any residual oil that did not burn in the fire from migrating into the watercourse. The impacted soil, brush, and weeds will need to be scraped from the area and be removed. I advised them to contact the surface owner of the property to determine what they would like done with the charred trees. They will remove the tank after speaking with the farmer to avoid any unnecessary damage to the corn. I will check back later to monitor site clean-up.
0999643817 2013-10-09 Production Wells No Violations I was on location to check the status of the rebuilt tank battery and surface equipment. The well has been turned back on and the proper ID has been posted on the 300 barrel stock tank. The dike has been rebuilt and meets SPCC specifications. The battery contains the stock tank, a 50 barrel poly tank, and a horizontal production unit. The well is operating with a plunger lift system. All the areas affected by the lightning strike and fire this summer have been reclaimed. No violations.

1375299928 2013-07-15 Production Wells No Violations I was on location this morning to check the status of site clean-up following a tank explosion on 7/10/13. W. Pol Contracting was on location scraping the impacted soil along the north side of the location. The diked area that contained the tank and separator has been cleaned out and the wellhead is being rebuilt with a new plunger lift system. A temporary tank was inside the dike and was used to contain fluids during wellhead repairs. Almost the entire impacted area has been bladed and the soil has been stockpiled on the south side of the location and mixed with lime for removal. There is no visible free oil in the stockpile. The siphon dam was still in place and functioning. There was an area of impacted grass at the east side of the location in the drainage area that I requested be removed. This clean-up continues.
1615025468 2013-08-08 Production Wells No Violations I was on location to inspect the clean up status of this well site following a lightning strike on 7/10/13. A new 300 barrel tank has been spotted inside the dike as well as a 50 barrel poly tank. The well is shut in and all the equipment has been rebuilt. The stockpile of cut trees has been removed and the affected area inside the woods has been mulched and seeded. There is no ID sign on any of the equipment and I will inform Equity to install the new ID sign before placing the well back in production. The site clean up is complete. No violations.
1821335977 2018-12-27 Production Wells No Violations Status check inspection of this Clinton Sandstone well. My inspection found the wellhead equipped with 1.5 tubing and a plunger lift system. This well produces to a diked tank battery on location that is shared with the Sobwick #2 (PN#20801). The battery contains one (1) 300 barrel tank, a 50 barrel poly tank, and a horizontal separator. There is a domestic tap and meter on the separator sales line and the chart at the separator indicates a differential production pressure of 50psi. The required ID is posted at the tank battery and the wellhead is identified. No violations.
