0696226529 2014-01-08 UrbanProduction Wells No Violations I inspected this well to follow up on Chiefs Order #2013-136 issued on 9/25/2013. The operator, Stephen J. Svetlak, LLC, has failed to maintain proof of liability insurance as required by ORC 1509.07(A) and has been found in material and substantial violation for failing to maintain coverage. My inspection found that the well was still in production and is in direct violation of the Chiefs Order to immediately suspend all oil and gas operations in the State of Ohio and 1509.07[C] of the Ohio Revised Code. I documented that the well is producing into a Dominion/East Ohio Gas line. The chart was changed out by Tri State Measurement on 1/07/14. Records indicate that no commercial production has been reported for this well since 1992. The well is operating with a plunger lift system and produces to a horizontal separator and 50 barrel steel tank. There is a domestic hook up on the wellhead and a gauge indicates that there is 60psi tubing. There is no gauge on the 3 casing. The tank battery is diked. The required identification is not posted on any of the equipment.
0778121756 2017-05-16 UrbanProduction Wells No Violations I inspected this well location as a follow-up to Chiefs Order # 2016-171. At the time of this inspection, I observed the following: a wellhead with plunger-lift system and domestic metering connection at the 4.5-inch casing. The connections and fittings at the wellhead were in good condition and well maintained. The battery was comprised of one 25-barrel steel, stock tank and horizontal separator. The well and battery were located on a parcel utilized otherwise for agricultural purposes. The well was in production and the charter recorder was active and in-use. There was no identifying signage at any place on the well system. I did not observe any signs of impact to the surrounding areas. The access road from Newcomber Road was mostly grass-covered but in passable condition. Per my discussion with Mr. Svetlak on 05/16/17, the well was in active production at the time of this inspection. There has not been an annual production report filed since 2001.
0981022014 2018-01-02 UrbanProduction Wells No Violations Follow-up to Chiefs Order 2017-441 to determine if Stephen J Svetlak, LLC has suspended all oil and gas operations in the State of Ohio as required by the Order. I was on location with Northeast Regional Supervisor Tom Hill to conduct the follow-up inspections. The inspection today found that the gate valve on the tubing has been closed and that the valve on 4.5 production casing flow line remains closed. The chart indicates that the well is not currently in production and this well is in compliance with Chiefs Order 2017-441.
1539781227 2018-10-17 UrbanProduction Wells No Violations Follow-up to Chiefs Order 2017-441 for failure to maintain liability insurance and to determine if Stephen J. Svetlak, LLC is in compliance with the order to suspend all oil and gas operations in the State of Ohio. My inspection today found the well equipped with three (3) inch production casing, 1.5 tubing, and a plunger lift system. The gate valve below the lubricator is open and the tubing flow line valve and main flow line valves are open. A gauge on the lubricator indicates a production pressure of 550psi and the port valve on the casing is open and supplying domestic gas. The tank battery consists of one (1) 70 barrel tank and a horizontal separator. The inlet and outlet valves at the separator are open and the meter chart was changed two (2) days ago and it shows that the well has cycled twice and is selling gas at a differential production pressure of 30psi. It is determined that the well is in production in violation of Chiefs Order #2017-441. Photos are available upon request.
1867934326 2017-12-27 UrbanProduction Wells No Violations Follow-up to Chiefs Order 2017-441 to determine if Stephen J Svetlak, LLC has suspended all oil and gas operations in the State of Ohio as required by the Order. I was on location with Northeast Regional Supervisor Tom Hill and Mahoning County Inspector John Fleming to conduct the follow-up inspections. The inspection today revealed that the inlet and outlet valves at the meter run (sales point) are open. The chart is currently recording gas production from the wellhead and the gate valve on the plunger lift system is open. This well remains in production and is not in compliance with Chiefs Order 2017-441.
