2446961 Primary Facility 2016-02-02 Incident- Response to Accident or Event Viol(s) Noted & Immediately Corrected The Department inspected the SWEPI LP - Stanley 1106 (ESX10-117-0243) located in Osceola Twp., Tioga Co. in response to notification of a spill by Patrick Bernethy, Environmental/Regulatory Tech from Shell Appalachia. Mr. Bernethy contact the WSQ @ 1150 hours on 1/26/2016 to notify the Department that SWEPI LP removed the temporary containment liner used during well completion activities on the 23 H well (API#117-21792) at the Stanley 1106 pad. As part of SWEPI LP’s standard procedures, soil conductivity readings were collected from the pad following the liner removal. Soil conductivity measurement points exhibited readings above background in the area of the temporary liner; approximately 6-8 mS. SWEPI LP was not notified during completion activities of any spills by the completions contractor, US Well Services.
2481524 Primary Facility 2016-04-28 Administrative/File Review No Violations Noted 4/28/16: The DEP has received a Remedial Action Completion Report noting the following:

On January 26, 2016, while conducting an environmental assessment following well development activities at the Stanley 1106 well pad, SWEPI personnel identified an area of the pad exhibiting soil conductivities readings above background. Shell notified the Department of its findings that same day and elected to remediate the area affected area. SWEPI LP contracted Matt Bailey (AMEC Foster Wheeler) to conduct an environmental pad screening from 1-26-16 to 1-28-16 for both well and staging pad, screening was also conducted on drainage ditches and the sediment basin to the north of the well pad. Impacts were not detected beyond the gravel surface of the pad. From Jan. 27, 2016 through Jan. 29, 2016 approximately 800 tons of impacted gravel were excavated and disposed of at the Waste Management Landfill. 2487541 Primary Facility 2016-05-26 Routine/Complete Inspection No Violations Noted he Department of Environmental Protection (DEP) inspected the SWEPI LP – Stanley 1106 site, in Osceola Twp., Tioga Co. on 5/26/2016 @0950 hours, in response to notification by SWEPI of suspected vandalism. The weather was mostly cloudy 75 degrees with winds S 7mph. SWEPI Env. Coordinator, Pat Bernethy stated the production tank that was installed for the 23H well appeared to have a faulty gasket around the heater. This had allowed production fluid to seep into the permanent causing elevated conductivity in the accumulated fluid. The gasket was repaired and the fluid was removed from the containment. According to Mr. Bernethy ~1-2 weeks later a small area of damp soil appeared to be coming from underneath the permanent containment. The gravel inside the containment was removed and the liner was inspected for holes. Approximately 8 puncture holes and 1 large gash were observed along a 5 foot length of containment. 2542025 Primary Facility 2016-05-09 Administrative/File Review No Violations Noted The Department of Environmental Protection has reviewed the April 20, 2016 document titled “Remedial Action Completion Report” for the SWEPI – Stanley 1106 Well Pad prepared by SWEPI and submitted to the DEP. The DEP has reviewed the subject document in accordance with the Alternative Remediation provision of the department policy document 800 – 5000–001 addressing spills and releases at oil and gas Well Sites or Access Roads. No technical deficiencies were noted.
2553030 Primary Facility 2017-01-18 Routine/Complete Inspection No Violations Noted The Department of Environmental Protection (DEP) conducted a routine inspection at the Stanley 1106 well site located in Deerfield Twp., Tioga Co. on 1/30/2017. The weather is 29° and cloudy northwest winds 13 mph. Three wells wereobserved drilled on the pad. Two wells appear to be in production. A large puddle near the well heads and some production equipment has a field measured conductivity of 150 microsiemens/cm. A new ESCGP-2 has been issued for the site to facilitate a pad build-out complete with installation of PCSM features. Cut and fill slopes appear stabilized by >70% perennial vegetation. 2855599 Primary Facility 2019-03-19 Routine/Complete Inspection No Violations Noted The DEP conducted a routine inspection of SWEPI's Stanley 1106 Well Pad in Osceola Twp, Tioga County on 3/19/2019 at 1145 hours. The weather was partly cloudy and 41 degrees. The act 9 sign was observed at the site entrance. The E&S Plan was in the mailbox by the entrance. The inspection log was in the mailbox by the production tank and company personnel last inspected the site on 3/16/2019, noting no issues.

There is currently 2 producing wells onsite. The 2H is a Marcellus well and the 23H is a Utica well. The site is very well vegetated and stabilized. The reclaimed pony pad to the north was hardly noticeable. No erosion was observed onsite. Containment structures were intact and no leaks or spills were observed. PCSM BMPs were installed per the site plan.

No violations or issues noted. I departed the site at 1215 hours.
